Nuking the Polar Caps is a Special Project unlocked at the planetary level. It requires a rocket which is consumed in the process and releases water to help increase Terraforming. Once the project becomes available simply choose an available rocket to send and after the Project completes the achievement will unlock.

To do this, you need to research the 'Nuclear Terraforming' technology in the terraforming tech tree. Thereafter, new planetary projects will appear as "Melt the Polar Caps". Complete the project to unlock this.
