You need to successfully perform a QTE even during a fight with all party members. I suggest doing this when you're fighting a boss; they have the most HP and the chances of getting a prompt for Combo attacks are higher.
There are accessories that raise the chances by 5%: "Muse - Yin" & "Muse - Yang". You can equip 1 on the character you use in fights, and the other on the one you want to perform a Combo with. The rest is about luck (RNG).
To have a chance to perform a combo you need to use
or attack near the character you need the combo with but you have to rely on RNG to get a chance.

It's mentioned in the comments that spamming also works which is a much easier way of trying it, thanks @Za Pantsupati, his comment below:
"I also just spammed X attacks and still got combos, so you don't need to cap them off with a heavy attack. That also ensures you never miss the combo QTE since it's always the X button to trigger it".
I got this through natural play, as long as you switch the character you play regularly you shouldn't find it a problem.
If you are too strong for all enemies, you can lower your attack by equipping weak weapons and then entering the fights.
Combo combinations:
Yue Qingshu > Bai Moqing or vice versa
Yue Qingshu > Sang Yo or vice versa
Yue Qingshu > Xiu Wu or vice versa
Xiu Wu > Bai Moqing or vice versa
Xiu Wu > Sang Yo or vice versa
Bai Moqing > Sang Yo or vice versa
Additional information that is helpful from the comments by @DecayingDestiny, he says:
"One more bit of info is if you're like me and missed this achievement during the main game you can talk to Arong at Mingshu Sect after the story to access time trials.
I farmed Duzhang Qi'ou to get the last one or two. The achievement popped after restarting when the game froze on a loading screen".

You also need to trigger a combo between Sang Yo and Bai Moqing. Achievement unlocked as soon as I triggered their combo. You likely Yue Qingshu and Xiu Wu as well, they were the first combo I had early on.
X-X-Y also works to trigger the combos.

Yeah, Foggy's guide is missing two combos. Full list should be:
Yue Qingshu + Bai Moqing
Yue Qingshu + Sang Yo
Yue Qingshu + Xiu Wu
Xiu Wu + Bai Moqing
Xiu Wu + Sang Yo
Bai Moqing + Sang Yo

A "combo" in this game occurs completely randomly in battle. You will see a large X show up on screen which means you should hit 
to initiate the combo. You will likely be mashing to do you basic attacks, so it is unlikely you'll miss the prompt, but I suppose it could be possible. Once successful, you'll see the two characters stand side-by-side and unleash an attack on the enemy. The game doesn't track this for you in any way, so whenever one happens, be sure to take note of the characters so you can cross it off your list.

It doesn't matter which character you're controlling in the combo, so for the first one below if you're controlling Bai and get paired with Yue, it counts just the same as if you were controlling Yue and got paired with Bai. If you don't get this done by the end of the game, use the function to replay boss battles by talking to the creature in the video below.
