I did it with the doctor and it didn't unlock, listened to all choices for all the different parts of the test.
On my first run through, i only noticed it available once - when you speak to the young yurlkle guide when at the Olympic village. So not many options to do it.

Not sure if this one is glitchy or not As I have listened to the doctor, olga and now the detective when he arrests you and still didn't unlock

The trophy is obtained at the very beginning of the game, during the first conversation with the doctor. Consider both answer options for each question. When thinking about the first option, I came across the fourth question
