In co-op, if all your teammates get downed, you get a message saying Last Man Standing. If you can successfully complete the mission after seeing this, you'll get this achievement. You can revive your teammates and be downed yourself, as long as you win the mission at some point after seeing it.

A team savior score is earned when you are the last man standing and you successfully reboot (revive) any fallen team member. Complete the mission after earning this to unlock the trophy.

"Team Savior" is awarded for reviving your team as the last man standing. Simply let your team fall, reboot them, and complete the level.
