When you defeat two minions of the last boss, he will jump off the elevator and throw a lot of grenades, you just need to press RB to slow mo and defuse grenades in the air...
You can get about 100 in 15 minutes - very easy achievement
Just not shoot at him, pick easy difficult and add all points to health and health regeneration.

Make a private coop match on Expert in Scandinavia, use a sniper or the Kusanagi. Fight your way past first checkpoint. Shoot the riot shields through the wall with Dart Overlay, but don't kill the guys on top of containers. A container might pop some guys on the floor, kill them too. The trick is when you come out of the checkpoint door there will be some wooden crates on the right side, sit down with your back to these crates. The guys on top of the containers will throw grenades at you. It will be raining grenades, just breach the ones that lands in front of you, there's 4 guys throwing them constantly so just breach away.
For extra fun, equip your grenades before you start running, and throw them into the group of enemies as you run by. Only recommended if you have unlocked the perk that allows you to take no damage from your own explosions.
This cuts the run to the first checkpoint down to nearly nothing, and allows you to respawn those grenade-throwing enemies quickly if they aren't spamming you with grenades like they should.
Shout out to Mr Rodster for pointing this out to me. And, by the way, this achievement sucks. Who thinks crap like this is a good idea?
How do you get more to spawn?
If you are just getting bombarded with Molotov's and hardly any grenades, just restart checkpoint and try again. Took me 2 and a half hours to breach 873 grenades.... If you get bored and want to save, there is a checkpoint just past the turret and through the door up the stairs.....
Load up Single Player Chapter 15: Kris on Easy.
Spend the points on whatever increases your health and the duration of DART.
Make your way through a few enemies and get to the battle with Kris. Keep shooting Kris until his health is at the halfway mark. This will trigger a cutscene where he detonates all the EMP mines around the room permanently and then beats you with his gun. You must Breach/take his pistol.
From this point on, Kris will will run around the area and only throw grenades because he doesn't have a gun. He turns invisible, but he's pretty easy to spot. His usual pattern is to throw 2-3 EMP grenades, and if those successfully take out your DART system, another volley of 3 frag grenades.
The best strategy here is to keep relatively close to him while also keeping an open space behind you in which to back up when he tosses the EMP grenades. Keep in mind that you have to be able to see the grenade to Breach it. When close, he'll usually toss them at your feet, so you can hit RB and go into DART and while backing up, immediately press LB on all of the grenades in the cluster (you need to press and release LB with each grenade). If the EMP grenades take you out of DART, run away from wherever you're standing because 3 frags grenades are coming at you and you can't Breach them. Even on Easy, getting hit by all 3 will likely kill you.
The drawback is that while he sits in some spots for a little while, he never stays in one place for very long. So you still have to put in effort to move around to keep him maintained at the perfect distance.
It takes about an hour to do depending on how many grenades you've Breached up til now. This strategy is less stress than the other solutions suggesting soloing Co-op on Expert, taking down Merit's bodyguards or dealing with him and his gun shooting you constantly.
"Breaching" a grenade disarms it, so naturally you will have to wait for an enemy to hurl a grenade to you. Just aim at it and hold . Disarming a grenade is extremely fast, and sometimes you can finish breaching before it even hits the ground. After a grenade is tossed, you have around 5 seconds to disarm it and subsequently loot it for your own use. There are two bosses you can grind this on: Kris, and the final boss.
Kris (Ch. 15), when in the second stage, will begin to toss EMP grenades. If you take him to a sliver of health, he will throw three at a time. He will run and throw a set, and if you managed to not get EMP'd, he will walk a few feet and repeat. On easy mode, it's impossible to die, even if you mess up.
The final boss on Chapter 20 is the other boss you can grind this on. After you kill the twins, down final boss' health by a bar or two so he's dropped his minigun and just run around the area disarming the grenades he shoots out every so often. This is also recommended doing on easy mode.
Grenades can be breached by holding the button when prompted to do so. 873 breaches is a lot and you will only achieve a fraction of that in the Campaign or Co-Op modes. There are 2 ways to grind this:
Campaign: In Chapter 15 during the boss fight with Kris, all he ever does is throw a multitude of grenades at you. Replay the chapter on Easy difficulty, position yourself in the Kitchen area to the left, and let the grinding begin. Kris will typically throw 2-3 EMP Grenades at any time, followed by 3 Frag Grenades if you fail to breach the EMP Grenades when they go off. Over time, this method can be pretty annoying as EMP Grenades render you helpless if they do go off and it is time wasted waiting for the effects to wear off. I recommend obtaining the chip upgrade which reduces the amount of damage from explosions. (Credit to broodwars).
Co-Op: In Scandinavia, in the first open area where an APC with a turret appears, one of the soldiers is a grenade-happy fellow who will keep throwing grenades at you. You need to position yourself a certain distance from him. When you do, all the enemy soldier does is throw frag grenades one after the other in quick succession, allowing for easy breaching. You will also find more enemy soldiers with this behavior further on in the same map. I found this method to be quicker than the campaign method. (Credits to pZiuz).
Remember to progress to a checkpoint to have the game save your latest stats if you intend to stop grinding before unlocking the trophy. This stat can be tracked in the Main Menu > Extras > Player Statistics, next to "Grenades Breached".
