To accomplish this you need the upgraded Target Identifier add-on version 3.. this takes energy so I would suggest doing it near a power station.
1. YOU CAN NOT BE SPOTTED - the target can't be hostile. Use Stealth.
2. I would suggest using a slow moving target such as the robotic spiders or the Mr. Handy (fallout) looking floating robots.
3. The same target CAN be used over and over so my method was to Hack.. wait for them to be scrambled, then Idle, then repeat.

You can actually get this with Version 2 - I got it early on Flight Deck by hacking a Cyborg Assassin repeatedly around the corner.

You can actually hack the same camera over and over to get this! I just got credit for it. It'll complete the hacking bar and then go back to the start as if you haven't hacked it. But you can repeatedly just do the same one. Give it a go, you'll be done in a minute!
