When you die (which will happen quite a lot, especially when you are starting out), you can either "Take a Break" which sends you back to the main menu, or "Try Again". Pick the Try Again option, get your blessing (I recommend more health potions or Wyrdcandles) and then choose a class to get this achievement to pop

This achievement also pops if you succeed. My first run was a success and it still popped when you choose "try again". So it's less about the dying part and more about choosing "try again" on the game end screen (whether you get there via win or lose).
