This achievement can not be missed. It is part of the natural story progression. There is absolutely no need for a guide to complete this achievement.

Story related, cannot be missed.
After you remove General Pollux's eye whilst playing as Fiona you will then take control of Rhys. You will then start talking to the ghost of Handsome Jack. You will then stumble off the walkway as Handsome Jack tries to attack you. When you wake up, your friend Vaughn will start talking to you and the trophy will unlock.
