This is the fourth story related achievement in the DLC for finishing the second mausoleum. The mausoleum is in the Shinefall Woods near Thistlym and Pelegion. The achievement pops after beating the boss. Your party will seem to be wiped out when the bosses health gets low enough, but it's part of the fight and you will be alive with low health after the cutscene.
