This achievement requires you not only to encounter every enemy in the game, but also fill out their respective data pages to 100% completion. There are 481 total monsters to Magic Lens. Note that just because a monster is in your book, it does NOT mean its data page is 100%. Only monsters who you encounter AND magic lens count for the achievement! If a monster has any ??????? in their stats it means you encounterd the monster but failed to Magic Lens the monster, so it DOESN'T count. You'll have to go back, fight it AND magic lens it to register it completely.

Note that story bosses are no longer missable, as they were in the X360 version. You can choose to replay most bosses for their Secret Missions or monster entries at Nam Cobanda Isle if you happen to miss a boss.

To check if an enemy has been Magic Lensed or not, press cn_RB (or whatever button you mapped to re-select an enemy). If you cannot see the health in numerical value (ex: 50,000/50,000), then you have NOT magic lensed the enemy

The most common missed enemies are, in my experience:

1) Adecor and Boccos in their first encounter with Yuri at the beginning of the game during the tutorial. Believe it or not you can magic lens them (most people miss this because they are new to the game and it isn't exactly the first thing you do when you are learning basic controls).

In fact, in every encounter you have with them, you can Magic Lens them for an entry. These guys don't come back until a New Game, so you MUST get these entries the moment you encounter them!

2) Zagi, in his first encounter, has two "forms": you can Magic Lens him prior to Estelle joining the battle AND after she joins the battle. They count as two separate entries.

3) Duke's Third Form "Radiant Winged One" is only unlocked if you "beat" the game with all the Fell Arms obtained. After slaying his second form, the Fell Arms will combine into him and unlock his third form. This encounter is missable if you fail to collect all the Fell Arms, but can be revisited once you do.

4) Traitor to Heaven, and every unique enemy in the EX dungeon "Labyrinth of Memories" This superboss & enemies within is part of an EX dungeon that requires you to revisit the Crag of Pharaoh prior to going to Zaude. There, you will see a portal that will take you back to the mirage of Yormgen. Complete the dialog events and leave the Crag of Pharaoh, complete the game and the EX dungeon will unlock after you return to the Illusion of Yormgen & go to Danghrest after visiting the Illusion again. This is an EXTREMELY easy to miss dungeon if you are not careful and is required for unlocking several entries in the monster book to complete it.

5) Gauche and Droite & Coliseum entries. There are unique enemies in the coliseum to fight and you need the five challenge letters to unlock the full potential of the 100 man and 200 man melee modes to get every unique enemy.

For the Hunting Blades letter, sometime after the final dungeon appears, go to Keiv Moc and a scene should happen right when you walk in, you should get a title for Karol and a letter.

For Leviathan's Claw, once you've completed Zaude, head to Torim Harbor towards the buildings just up the stairs from the docks. You'll see a scene with Gauche and Droite. After the scene, head to Ehmead Hill and enter it from the left side. Head up and to the right to get a scene with the party and obtain the Fire Lily. Next, head to Mt Temza for another scene to trade the Fire Lily for the Compact of Remembrance. Finally, head to Zaude again to the room where you fought Yeager, and you'll fight Gauche and Droite (one time encounter) and earn a letter.

Flynn has a letter you earn by going to Zaphias Lower Quarter and sleeping at Yuris room after Aurnion is constructed.

Clint has a letter you can get by going to Caer Bocram after Aurnion is constructed. Go left into the area on the ground floor where the trap door is for a scene and a fight.

The fifth and final letter is unlocked after completing the EX dungeon "Labyrinth of Memories" (mentioned in #4).

You need ALL five letters & 100 man melee done on 3 characters to unlock 200 man melee.

Additionally, you need to run the 100 or 200 man melee with Yuri, Flynn, Karol and at least one of the other members because specific enemies will swap out and be replaced with one off enemies that you need to magic lens.

Finally, you need to run the Tag Team challenges in the Coliseum after you complete the 200 man melee with at least one character.

You need to complete the following tasks in Tag Team to unlock every entry:
-- Raven must not be in the fighting party (can stack with Karols).
-- Karol must not be in the fighting party (can stack with Ravens).
-- Each of your party members has an entry for running the Tag Team Savage mode without them in the active fighting team. You need to swap out your fighting team at least once to register all the entries (do a run with your four regular members in the fighting team, then run it again without any one of them in the team).

6) Necroplolis of Nostalgia is unlocked after everyone gets back together in Arc 3 of the game after Zaude. Go to Zaphias Castle into Estelles room for a cutscene. Then return to Zaude in the room you defeated Alexei in to unlock the bonus dungeon. You can only do the first 2 sections before you beat the game. After you beat the game, return and do all 6 sections and Magic Lens all the unique monsters here.

If you complete the game you can transfer your Enemy Book for 10 GRADE and focus on the entries you missed in New Game+ playthroughs. It is possible to get every enemy in one playthrough, though it is more advantageous to do so in your run with GRADE modifiers while hunting items for the collect every item achievement/going for level 200 achievement.

So I have 481 monsters, I'm on my second playthrough and my collection book has me at 99%

Any suggestions? I've combed all the lists I can find and unless they've added monsters from the ps3 version to this one, then I definitely have everything. Does that final bit trigger upon completion of the quest(post game)?

First playthrough done. Missing one enemy.

When you're in Helilord, and get access to the lower quarters, save before you go down. There are scripted fights there, but if you go to the left instead of right, you probably miss the same knight as me as you get one fight less.

This isn't completely true. The one boss that I missed on my first playthrough was Baitojoh during his Karol-only phase. When I replayed this battle at Nam Cobanda, the fight started after this phase, so it will be playthrough number two before I can get it.

Does Repede's Inspector skill take care of lensing while he's in the party?

Finally found a regular normal guy that I hadn't lensed. Repede didn't seem to scan it with his skills on. The only benefit to him seems to be showing health bars (which helps to show what you've missed) and FULLY scanning every unit on the field with 1 lense.

@GreatPotBandito, I see you've unlocked the achievement now, but I freaked out when I hit 99% and all my counts matched as well. I realized the count at the top of the book only shows the number of monsters you've fought/seen, not necessarily that you scanned them along the way. Somehow, I had missed scanning a common enemy at Releweise Hollow and its stats were all "???" so I simply had to go scan it. Scroll through the book and look for question marks if you're m

You will get the Monster Book once Karol joins your party. Every monster you encounter before and after that point will be put in there, but their pages will not be filled out unless you use a magic lens on them. You can only carry 15 at a time (on your first playthrough), so be sure to stock up every single time you come across a shop. If a boss goes through various stages, you may have to scan it more than once.

Hold down the  to see if an enemy has not been scanned (also do this when a boss changes forms or after a cutscene mid-battle); you will know it has not been logged if its HP is not shown. If you already have it in your book, it will show the HP of the monster. If you encounter a boss-type character more than once throughout the game (the two idiot knights and Zagi for instance) you must scan them every time you encounter them. Be sure to buy the option in the Grade Shop to carry your book over to any future playthrough to continue working on this if you miss something.

See this thread for all the monsters you need and where to find them, and be sure to take note of ones that are missable.

When you reach the end of the Necropolis of Nostalgia, be sure to change the system language to English before the final boss of the dungeon. Due to a Russian localization bug, one of the parts of the boss is shown as Chroma, and because of this it does not appear in the book (and you won’t be able to fight him again, only NG+).
