How to unlock the Back Up Plan achievement in Tales of Vesperia - Definitive Guide
Here we go. Save at every single save point in the game.
There are 57 of them. The bad news is that 19 of them will disappear or become otherwise inaccessible at some point throughout the game. The good news is that during your travels, you are guaranteed to run right past all but 3-4 of them, and there's only a couple you really have to look out for. With a little bit of planning, this becomes a very easy achievement.
A few notes:
-This all must be done on one playthrough. The save points you have used do not carry over through EX New Game.
-None of the save points are really out of the way or time consuming to reach. You can very easily do this achievement on your fast playthrough (for the Speedster achievement) with no difficulty.
-It seems that technically you don't have to save at every point, just physically touch them all. The achievement seems to unlock just upon touching the last save point. I'd still just save each time to be sure.
Here's the full list of save points, listed in story order. The ones that disappear are in bold. I've also gone ahead and picked a few that I think you might accidentally miss and underlined them.
Zaphias
1. Right outside Yuri's house as the game starts
2. As you leave your prison cell.
3. Just outside Flynn's room.
4. Royal Quarter.
Deidon Hold
5. By the shop wagon.
Quoi Woods
6. In the middle clearing, by the blastia.
Halure
7. At the inn.
Aspio
8. In the library. Becomes unavailable near the very end of the game.
Shaikos Ruins
9. In the room with the boss.
Ehmead Hill
10. On trail, right before the boss.
Capua Nor
11. At the inn.
12. As you exit Ragou's mansion.
Capua Torim
13. At the inn.
Caer Bocram
14. In the room with the spiral staircase.

28. Next to you after the cutscene with the parents. You'll see it before but can't use it until after the scene.
Yormgen
29. At the inn. Don't miss this! It's not exactly out of the way, but this is only available during this short story section.
On the Fiertia
30. In the cabin. It seems you get two chances at this one: now, and later when Judith passes out. I'm pretty sure it's the same save point, but save both times to be sure.
Manor of the Wicked
31. On the second floor.
Mt. Temza
32. After crossing the bridge and meeting Judith.
33. On the path up to Baul's cave.
(30. Reminder to save in the cabin of the Fiertia. Last chance.)
Egothor Forest
34. In second area, at a three-way split in the path.
Myorzo
35. In the resthouse.
Baction
36. BF1, on the bridge.
37. BF2, on the first bridge.
38. BF2 (second time, after the floor maze on BF1), on the second bridge.
Heracles
39. On the bridge after you lower the colored gates.
40. Right outside the engine room's doors.
Blade Drifts of Zopheir
41. Right before the aer krene/boss.
Zaphias
42. In the Dining Hall. Slightly tricky, because this save point isn't here when you're here at the beginning of the game. Save now!
43. In the large room with the locked door.
44. Outside, at the bottom of the spiral staircase up to the boss.
Shrine of Zaude
45. On the shoreline, as you enter.
46. Two rooms after the first water puzzle, before the first boss.
47. After the second water puzzle, before the second boss.
Blade Drifts of Zopheir
48. Near the aer krene. It's moved, so it's a different save point!
Erealumen Crystallands
49. As you meet Nan.
50. As you come to the aer-filled floor by the boss.
Relewiese Hollow
51. As you meet Duke.
52. Just outside the cave.
Northern Hypionia/Aurnion
53. In the field after using Brave Vesperia #1. This is the save point that people miss most often. You get one chance at this. When you do the next, very short story part and come back, it's gone.
54. Upon your return, when Aurnion is built.
55. When Aurnion is fully built. This is a sidequest that can be done at any time, even after you beat the game. Talk to the knight outside the Aurnion HQ, and he'll send you on a few fetch quests for various materials to upgrade the town. Do a few of these, and the last quest will simply be a request for 500,000 Gald. Give him the money, rest a few times, then come back. There'll be a new save point by the inn, but Save Point 54 will disappear.
Tarqaron
56. Solve the rolling ball puzzle, then continue outside.
57. At the bottom of the very tall staircase.
The most commonly missed ones seem to be 19, (don't forget to go back into your prison cell in Ghasfarost), 48 (second time through the Blade Drifts), and 53-55 (Hypionia/Aurnion).

Basically save at every save point in the world.
Important things to know:
Aurnion (town later in the game) has three save points, the little field, the actual town, and then the upgraded town.
Tarquaron has one save point at the end of the dungeon before the final boss.
One save point commonly missed is the one located in the Blade Drifts of Zaphier. This save point is missed the most as the cutscene for the spirit interferes before you can go up and save on it.
Important things to know:

It's also important to note that almost all of the green save points (the ones that heal your party) disappear later on (these mainly appear right before boss fights) so make sure you save at every single one of these.
There is also another save point in the Blade Drifts of Zophier which appears if you visit it later on. It's in the same vicinity of the previous save, but since it moved it counts as a different save.

Save points are yellow/purple orbs with a book in the middle, or some are all-blue orbs that also restore HP/TP. Some are missable, so make sure you follow the guide if necessary. There really aren't any that are "hidden" so just make sure you use every one you see.
