If you want to get this right away, you can just go to Chapter Select.

Choose Epilogue and play through it.

It's mostly skippable Cutscenes.

It didn't unlock it when I had finished the game, a couple of weeks ago. I had to replay the epilogue through chapter selection, as mentioned in this guide.

It didn't unlock immediately for me. I ended up having to play through the end sequence again after walking with Louis as an old lady again it unlocked

This trophy did not appear the first time along with the trophy for all the melodies. I missed cutscenes and minigames. So, just in case, it's better not to do this.
After replaying the final part of the game without skipping cutscenes and minigames, everything fell out.

After the credits and when you go back to your childhood. You should definitely visit the graves. After which the trophy will appear.

You can simply click on the menu to select a part - epilogue - skip the entire video - follow the npc - skip the video - trophy. Special tried it on the PS4 version, I got it
