How to unlock the Cowabunga It Is achievement in Teenage Mutant Ninja Turtles: Shredder's Revenge - Definitive Guide
Your combo count is displayed just under your health, and like many games, it builds as you hit enemies, you have a few seconds between enemies before it ends, and if you get hit it ends automatically. For this achievement, you need to string together 250 hits without losing your combo.
You'll likely get nowhere near this naturally on your first playthrough, but thankfully there is a very easy and reliable way to get this on the first episode of Story Mode. Basically, you can juggle a defeated enemy against a wall endlessly until you reach 250 hits. See the video below for where to do this in the kitchen area of the first episode. To sum it up, when the door opens on the right side of the kitchen, three purple foot soldiers come out and hold ice blocks to throw at you. Defeat two and leave one alive. Bait the last one to jump onto the counter on the left side of the screen, and you stand on the floor against the counter. Now just mash . The counter prevents you from moving forward while you attack, keeping you at the perfect distance to endlessly juggle that enemy. Full credit to the creator of the video below.

Getting a 250 hit combo can be pretty daunting in this game, particularly on harder difficulties. You can play through the first level on the easiest difficulty after memorizing enemy patterns, or choosing a level like 12 and farming mousers at the beginning of the level, avoiding the big mouser, but there's a much easier way of doing this, although it'll require some trial and error.
Start the first level with a character that has a high speed rating (I used Raphael). You may have noticed during your playthrough that you can continue to juggle enemies after defeating them, and that's how you're going to do this. At any opportunity, try to pin an enemy ninja against the left side of the screen, with you standing maybe a foot or two away from the edge. Don't use combos, but instead just tap at a steady pace until you're able to juggle the ninja in place. Make sure you aren't juggling them against a solid wall, as they will eventually pancake against the wall and explode.
