At the main menu, go to the Online tab and select Tekken Fight Lounge. Select one of the available options, for me it was the top one, Recommended (Europe). From there, go up to another person's avatar and press A. There should be an option for Ghost Match. Select this and win the fight. The achievement should pop after the match is over.

Achievement is flagged as Offline Only, but your guide says online mode is required. Is Fight Lounge the only way to earn this?

I'm not sure. I disagree with the flag, as this is how I unlocked the achievement, by going online. I did try fighting some ghosts before in offline but it wouldn't unlock for me. It's possible it's patched now but I don't have a spare account to test it, maybe if someone else comes across this later on they can add it here? I will adjust the guide if this proves to be true.
