When you die, if you reach that point in the level again, you will see there is a tombstone with your Gamerpic on it. You can pick it up to refill a heart. You need to collect three of these in a row.

The difficulty in this is that the levels/stages are random at each layer, so guaranteeing a return to a spot you died is a little problematic.

Here is how I pulled this one off:

In the layer you're on, you want to aim to not collect the eyes needed to proceed. Once you have enough eyes, you will not return to a level/stage you died on. Continue to die early on in the various levels of the layer and don't collect your past tombstones if you see them. They won't disappear and you can actually have 2 or 3 on a level within a layer. Eventually, you will have a bunch saved up in the various levels of the layer. Collect these and you will be able to easily collect 3 without dying.

Having extra hearts when aiming to collect will also make this easier to complete without dying.

I just got this and my experience was that it not keeps tombstones near. What I did in the end was to have multiple tries dying, and then have a perfect run in this layer. You will meet several of your tombstones, I got 5, If you fail, try in the next layer. Having multiple hearts will he you not die.

I got this by accident because I hit a glitch where I couldn't eat the enemies eyes at all so I ended up dying a lot and pretty much on all of the levels on that layer :D

Thumbs up and thankyou for the solution. This cheeve might be glitchy. I just died about 30 times on purpose then went for this cheeve. First tombstone I got this cheev popped. Maybe if you have 3 on one little level you can pick up it pops anyway.

i can confirm what has been said before... i was dying a few times to set up tombstones.. but accidentally picked up some.. after placing and dying i accidentally picked up a 3rd one and the cheevo popped.. WIN! :)

You can have one tombstone per screen so you can't just die in the same spot three times, but when your other tombstone is out of sight you can die and a tombstone will be there too. Basically you want to just die a lot and then once you have many tombstones laid throughout the different screens you want to get three without dying. This is best done in the early layers because there are fewer and less threatening enemies to get in your way of grabbing three tombstones. I recommend layer 2, Paranoia for this achievement because in addition to the Big Heads and blue Kamikazes, there are also Spitters to kill you in different places and they don't make going through the levels significantly more difficult.
