Level 1-6. Quickly transform two of the Tetras to form a 4-tile high staircase, then have the remaining Tetra climb the stairs and fall. See Baddriveri’s video around the 37 minute mark.

In level 4-1 you'll be taught how to die by falling - dropping more than 3 squares will kill you. To unlock this achievement, you need to die in a level before level 4-1. This is likely possible in multiple levels but I did it in level 3-4. Basically just build stairs and then jump off. The picture below shows how I set it up.
