Press LB while aiming at one of the nozzles below where Carnage hangs on the walls. This will light the whole floor underneath it on fire. Then, hold B while aiming at Carnage to hit him with a charge shot and knock him off the wall, he will fall into the fire. Simply do this 3 times and the achievement will pop.

Just wanted to point out you can stand where the fire spawns and turn on the flames when Carnage enters the area to pop the achievement as that is what I did at the very beginning of the fight because I didn't want to replay the whole level to get back there and I didn't know that he would climb the walls

I just unlocked after dying so it can be across multiple attempts - I had never beaten him before though

What I did was hit 
on all the nozzles at the start of the fight, then hold 
on him to knock him into the flames three times.
