The first escapee you'll find after the first few chapters, sitting on the floor right outside your apartment.
Later, after getting the spider-tracers (as mentioned by Darklights, you get them after leveling up to level 17) you will find the first set escapees.
Later still in the game after completing a few more chapters, the second set of escapees will show up. (along with the first set if not previously collected)
They are easy pick-up/drop off side tasks.
It's also semi-entertaining that the dialogue between the escapees and spiderman are completely unrelated!
My favorite banter -
Escapee: "Please don't take me back the hospital!"
Spider-man: "Just relax, I'm taking you back to the hospital."

Initially, you will not be able to see these side-missions on the mini-map, but after a certain point in the game, a Spider-Tracer upgrade will be unlocked. Purchase this, and you will now see blue people icons on the mini-map. Travel to each, pick up the asylum patient, and take them to the nearest police station. Do this for all of the patients and this trophy will unlock. You can monitor how many you need to find by pressing 
and checking the far right menu. Unlike the Infected Civilians, most of these are unlocked from the start of the game.

*Manhattan Side Quest Mission*
Escaped asylum patients are darted about Manhattan but are hard to spot until you get a specific 'Spider Tracer' upgrade in the Tech upgrades section. They will then appear on your map.
Take them to the front door of the police station or heli-pad.
