update October 18, 2021: The long wait is over; patch 3 (version 4.2.4.0) has been released on Xbox and it finally fixes the achievement!Playthroughs started before patch 3
If you have a save with a fully filled Codex prior to this patch, you only need to switch to a second character - make one if you don't have it - and then switch back to your main character. For me, it unlocked on the main menu but I did try to load up my character before switching, so you may need to load up your save to trigger it.Additionally, I found that the patch adds one (so far) additional codex entry, i.e. Habnet Technology under Manufacturers, which will be added when you obtain an additional Habnet item (e.g. Habdefender PPW from the weapons vendor). For many players, including myself, it does not seem to be necessary to unlock the achievement (at least not retroactively). However, if it does not unlock, you can try to buy the Habdefender PPW from a weapon vendor to add the new codex entry and then switch between characters on the main menu as Goatsaresuper commented below.
New playthroughs after patch 3 only
The codex has been changed for new playthroughs after the patch. Now, the codex has two extra categories, Factions and Species, and extra codex entries are added to Locations. These codex entries are added automatically at the start of the game, and I have included this information in the main part of the guide.Additionally, this achievement will now reportedly unlock much earlier in new playthroughs, well before filling the Codex or completing the story (thanks Owen Poff and Hieronimus). I don't know if this is intentional or whether this will be fixed in future patches, so pick up all codex entries just to be sure.

update October 7, 2021: Patch 3 is now live on Steam which should fix this achievement, but the first comments are not promising. Let's hope for the best when it hits Xbox and Windows Store.
update September 30, 2021: The devs put out a statement on Steam suggesting that the next patch will include the fix for this achievement (among others) and should be "deployed shortly". The new Xbox version will probably be released sometime after that, like all previous patches (thanks OniAegis).
update September 3, 2021: Patch 2 (version 3.0.6.0) is now available on all platforms, but unfortunately does not include a fix for this achievement. The devs reconfirmed in the patch notes that they are working on a fix and that the achievement will unlock retroactively (thanks UnmatchedBrace).
update August 19, 2021: Patch 1 (version 2.1.4.0) has been released which I assume coincides with a previous Steam update. Despite that the patch seems to fix the other Codex-related achievement Flatliner*, this achievement remains unobtainable and we will have to wait for the next patch.
* only if you were missing the last enemy Codex entry related to the game's final boss.
update August 12, 2021: I get many questions and comments about the patch, so I thought I would give some extra info. As of now, the latest patch for the game is not yet published (current version: 2.0.5.0) as it still needs to be approved by Microsoft (source). We won't know for certain which of the issues will be fixed by this patch until it is live (Steam patch notes), but it seems like that could be very soon. The patch after that will fix the broken achievements (source). Additionally, I am aware of the 0.01% of players that unlocked the Codex achievements on Xbox. This could indicate the patch will fix these due to internal testing, but we have to wait and see if this is indeed the case. I will update the solution when I know more.
update August 3, 2021: AchievingOutlaw got in touch with the developers who told him that they will verify if everything can be unlocked as intended and if not - patch it asap. So for now we still need to have a bit of patience for a potential fix, but the good news is that they will investigate this.

At this moment it is unclear if it's possible to unlock this achievement or how. I thought it would be helpful to get a list of Codex entries for people to compare to figure out what (if anything) is missing. I have completed the main and side missions, but only filled 86% of the Codex including 96% of the enemy Codex entries (as far as the progress bar is concerned). I assume the tutorial is filled automatically, so I have omitted that part.
Edit: As many have the same issue, we can safely assume that this achievement is unobtainable (or at least very buggy). For now, all we can do is wait for a fix.
Codex Entries
The codex is split into 8 sections, of which 3 are filled automatically at the start of the game, i.e. Tutorials, Factions, and Species. However, the sections Factions and Species are only present for new playthroughs after the release of patch 3 and are not required for previously started playthroughs. The entries of the remaining 5 sections are added during natural progression of the story and completing side missions. If you have completed all main and side missions, discovered the location Outskirts, and found equipment from all manufacturers, you should have all codex entries listed below and unlock the achievement.Locations (39) - (35 pre patch 3)
These codex entries are added when first entering these locations. If you have done all main and side missions you should have visited all locations except Outskirts, which is located east of Blossom Hills and north(east) of Black Lake Towers.Patch 3 adds 4 additional entries at the start of a new playthrough, and therefore will be missing (and not required!) when you've started your playthrough before the release of the patch.

On a good note they got the patch push to win10 pc properly executed this time 3.0.6.0 updated through the app without issue.
The saddest part about the whole thing is that this game was marketed as a co-op fun fest and that is the exact thing that broke most of the achievements for everyone.
I for one brought others in to the title specifically to play the game with friends. We did play through the entire game with each other however it created all kinds of issues and problems even outside of the failed achievements.
I really believe once they fix all these things this will be a wonderful co-op shooter. Hard to truly enjoy it when you are always being pulled out of immersion. Perhaps I should have waited 6 months just like with Cyberpunk to play it without issues.

if anyone has any idea where they might be, would really appreciate it.
My question is about when it IS patched, will it retroactively award the necessary codex entries and hence chevos, or are we likely to have to replay certain scenarios to get them? That's really my only worry, should people we holding off playing it at all for the moment or do you think it's safe to finish the game and assume that when they patch it, it will correctly give chevos for codex entries already obtained / or that should have triggered to be obtained already?
I love the game but won't be playing through the whole game again just to get a few codex entries so please let that not be the case!
Edit : ok i found the codex entry, you don't need to get a gear, you must pick up the codex page itself on a dead ennemy.
The following summary covers the key areas we have addressed in this new major Patch update for The Ascent on Steam.
This update is now available for The Ascent on Steam, and the same patch will also be landing for Xbox and Windows 10 players shortly.
Our team is continuing to work hard on improving the game and we are very grateful for all the support and feedback from our players.
Performance (PC Steam & Win10 only)
Fixes to improve performance in DX12, also with Ray Tracing.
Note to Players: on first running of the update, there will be a 20-25 second ‘stall’ on loading to Main Menu to allow some caching to happen before you play. This is a one time event.
Ray Tracing is now available to Players on the Windows Store.

Stability (all platforms)
Numerous instances of ‘one time’ crashes have been fixed in both Single Player & Coop on ALL Platforms. (some other instances remain, we are working hard to resolve these for the next update).
Local Coop fixes where Player 1 and Player 2 have controller focus issues on certain menus.
Numerous other crash-fixes have been made to improve the stability of both local and online Coop, including fixes when disconnecting controllers, issues when 3 Players start a new game, and fixes when the internet connection becomes unstable.
Fixed an issue where in certain circumstances a black screen would appear at the end of Mission 12.
Fixed an issue where sometimes it takes too long to return to the Main Menu after gameplay.
Fixed issues where Area loading is slower for the Client joining the game after the Host is using the Taxi.
Fixed some Weapon replication issues in Online Coop.
Fixed some issues around cross Platform hotjoining where Clients had trouble connecting to the Host.
Save Progress (all platforms)
Introduced a new system to circumvent situations where save files could become corrupted.
Fixes to situations in Coop where the Host leaving the game could cause the Client to lose progress.
Fixes to situations in Coop where Players may experience progression blockers in certain situations.
Fixed the issue where a Client's items would unequip after a Host disconnects.
Fixed an issue where a Client may lose connection when hotjoining a game in progress.
Gameplay (all platforms)
Fixed an Achievement issue where you could unlock Main Mission and Side Mission under incorrect conditions.
Fixed an issue where the mission objective during ‘Go To Grinder’ in Mission 2 would not update.
The issue where multiple bosses could spawn has been fixed. e.g. Papa Feral, Megarachnoid (Spider Boss), Gun-Dolph.
Fixed the Casino Courier Side Missions 1 - 5 where you could sometimes be unable to proceed due to missing NPCs.
Fixed the custom camera for ‘Dark Horse’ Side Mission.
Fixed the custom camera for Bartender in Stimtown.
Fixed the situation where sometimes ‘Dark Horse’ could not be completed due to the NPC remaining in a frightened state.
Fixed an issue where the Volatile Augmentation when used on neutrals had no effect.
Fixed an issue where the Siege Mech in the Prison could despawn after a Player died.
Fixed the issue where the Snooze Dealer’s Journal map icon was a white square.
Fixed damage scaling on the Pocket Mech minigun to make it more powerful as you level up.
Allows the use of certain augmentations while rolling.
Balanced cost/cooldown/efficiency of Life Transfer, Neutron Beam and Overclock
Fixed issues with PLUG Cyberdeck not cracking ICE 1.
Translation (all platforms)
Lots of fixes have been made to address instances where English would appear instead of the correct language.
Further work is continuing to improve the quality of the translations as well as further general bug fixes.
Fixed an issue in Japanese where text overlapped in the intro cut-scene.

Anyone else tired of developers releasing their broken/incomplete games, causing only frustration and headache for the paying gamer? You all should be.
I have little hope that ALIENS FIRETEAM will be released with no issues. I expect it to be derivative, and quickly become a grind. Of course, I'll still buy a copy. Urgh!

I loaded up my saved game and it automatically added the enemy codex entry and unlocked the achievement.
Comprehensive achievement is still not unlocked after getting Flatliner. My codex matches the authors list, with the addition of Interdimensional Calamity.
*Note* I am playing on PC/Steam
but nothing unlocked.

Just checking in to see if this game fixed its achievement issues... NOPE! Ok, I'll check back in about a month.
Until then, I'll check ALIENS FIRETEAM because their achievements are glitched even worse than this game

This happend to me, joined a friend's new game and it overwrote my completed game. When i log in now, I am basically a max lvl character in a brand new game. I doubt this patch will give me my save back...
Nevertheless, if you want to check yourself, Kudo is located in the Golden Satori casino, near the bartender if I remember correctly.
Although the Flatliner achievement remains stuck at 96% with the 438 Hung entry missing
It's funny because now I have Comprehensive unlocked and Flatliner still missing lol
Though, it sucks that Flatliner is not fixed for those with missing codex entries. Seems strange that it is possible to unlock an achievement for getting all codex entries but not the one with only the enemy codex entries...
I only had 5 out of the 7 codes entries for the Manufacturers but it still unlocked.

@Hooch Lee Were you missing Whisper Cybernetics and Habnet Technology? In that case, it kind of makes sense as these codex entries didn't unlock properly even when you did have equipment from those manufacturers.

Yeah, I think they just lowered the requirements, so everyone can unlock this achievement. It was probably easier to do than actually fix it, although that's just speculation from me. But like I said earlier, better that it unlocks prematurely than not at all.

Havoc is a flying combat vehicle of sorts. I think it's one of the boss fights in the main missions, right?

Anyway just like others switching characters after updating to the new version: 4.2.4.0 allowed for the achievement to pop and finally claim my 100%. Thank you for fixing it Neon Giant.

I am a little miffed by Flatliner still being wonky for some myself. Though I experienced the majority of the bugs reported across this game missing codex entry for Hung was never one of them.
I had neither the achievement for all enemies (84%) nor for all locations (72%)

Then I deleted my secondary Character (I just created one to see if unclocked by switching) , then after the character was deleted at the New Game screen the achivevement unlocked.

I was @80% with all codex entries unlocked. Created a new character, played for 15 minutes then deleted for achievement to pop.
