update December 2, 2021: Patch 5 (version 4.4.1.0) is released on all platforms which includes a fix for this achievement. Players who had issues unlocking it prior due to missing Codex entries related to certain bosses, can now reportedly unlock the achievement retroactively by loading up your previous save game (thanks TymanTheLong, Malboa77, and BRzLnWaRMAchiNe).Previous updates are under here for brevity
update October 18, 2021: Patch 3 (version 4.2.4.0) has been released on Xbox, and while it fixes the achievement Comprehensive it still does not seem to add any missing enemy codex entries for players who already completed the game.
update September 6, 2021: After the latest patch (version 3.0.6.0) missing entries are still not added to the codex for some, despite having finished the game. I have now completed 3 full SOLO playthroughs, and in each obtained all known/available codex entries, so I assume the patches have fixed the achievement for new playthroughs. Unfortunately, I don't know if a future patch will add any of the codex entries of enemies who you can only encounter once in the game. For the rest, I have added enemy locations so you can find them after the completion of the main story.
update August 23, 2021: This achievement - while not unobtainable - is still buggy after the latest patch (version 2.1.4.0): it is possible that some enemy codex entries, e.g. 438 Hung, are not added when they should, even with a new character (thanks Demon Slayer050). I recommend waiting for the next patch, which may add these missing entries.
update August 19, 2021: A patch (version 2.1.4.0) has been released which I assume coincides with a previous Steam update. The patch seems to fix this achievement*, but unfortunately not the other Codex-related achievement Comprehensive. If you have finished the main story, you will get the missing enemy codex entry just by loading the save. I have added this entry to the list below due to minor spoilers, but it corresponds to the game's final boss. Hopefully, everyone can now unlock this achievement. Let me know if anyone still has issues.

update August 12, 2021: I get many questions and comments about the patch, so I thought I would give some extra info. As of now, the latest patch for the game is not yet published (current version: 2.0.5.0) as it still needs to be approved by Microsoft (source). We won't know for certain which of the issues will be fixed by this patch until it is live (Steam patch notes), but it seems like that could be very soon. The patch after that will fix the broken achievements (source). Additionally, I am aware of the 0.01% of players that unlocked the Codex achievements on Xbox. This could indicate the patch will fix these due to internal testing, but we have to wait and see if this is indeed the case. I will update the solution when I know more.
update August 3, 2021: AchievingOutlaw got in touch with the developers who told him that they will verify if everything can be unlocked as intended and if not - patch it asap. So for now we still need to have a bit of patience for a potential fix, but the good news is that they will investigate this.
original notes: at this moment it is unclear if it's possible to unlock this achievement or how. I thought it would be helpful to get a list of Codex entries for people to compare to figure out what (if anything) is missing. I have completed the main and side missions, but only filled 96% of the enemy Codex entries (as far as the progress bar is concerned).
Edit: As many have the same issue, we can safely assume that this achievement is unobtainable (or at least very buggy). For now, all we can do is wait for a fix.*
* Enemy Codex is now fully filled due to the patch of August 19.
Enemy Codex entries
The Enemy codex entries are added when defeating your first enemy of a specific type or faction, and you will face each while doing the main and side missions. You should get all these entries naturally during your playthrough, with the last enemy typically the final boss of the game.Where possible, I have added locations where to find each enemy if you are still missing some entries after the main story is completed.

I wish I had more info for you. The best thing to do right now is to submit a bug report and hopefully they will fix it asap. I will update the guide when I know more.
Flatliner is the only achievement that I'm missing.
Either way, the devs are aware of this issue and as I mentioned in the update they are currently working on it for a future patch. So hopefully, that will finally fix it for everyone.

I got everything else from that list and my progress stands at 93%.

The following summary covers the key areas we have addressed in this new major Patch update for The Ascent on Steam.
This update is now available for The Ascent on Steam, and the same patch will also be landing for Xbox and Windows 10 players shortly.
Our team is continuing to work hard on improving the game and we are very grateful for all the support and feedback from our players.
Performance (PC Steam & Win10 only)
Fixes to improve performance in DX12, also with Ray Tracing.
Note to Players: on first running of the update, there will be a 20-25 second ‘stall’ on loading to Main Menu to allow some caching to happen before you play. This is a one time event.
Ray Tracing is now available to Players on the Windows Store.

Stability (all platforms)
Numerous instances of ‘one time’ crashes have been fixed in both Single Player & Coop on ALL Platforms. (some other instances remain, we are working hard to resolve these for the next update).
Local Coop fixes where Player 1 and Player 2 have controller focus issues on certain menus.
Numerous other crash-fixes have been made to improve the stability of both local and online Coop, including fixes when disconnecting controllers, issues when 3 Players start a new game, and fixes when the internet connection becomes unstable.
Fixed an issue where in certain circumstances a black screen would appear at the end of Mission 12.
Fixed an issue where sometimes it takes too long to return to the Main Menu after gameplay.
Fixed issues where Area loading is slower for the Client joining the game after the Host is using the Taxi.
Fixed some Weapon replication issues in Online Coop.
Fixed some issues around cross Platform hotjoining where Clients had trouble connecting to the Host.
Save Progress (all platforms)
Introduced a new system to circumvent situations where save files could become corrupted.
Fixes to situations in Coop where the Host leaving the game could cause the Client to lose progress.
Fixes to situations in Coop where Players may experience progression blockers in certain situations.
Fixed the issue where a Client's items would unequip after a Host disconnects.
Fixed an issue where a Client may lose connection when hotjoining a game in progress.
Gameplay (all platforms)
Fixed an Achievement issue where you could unlock Main Mission and Side Mission under incorrect conditions.
Fixed an issue where the mission objective during ‘Go To Grinder’ in Mission 2 would not update.
The issue where multiple bosses could spawn has been fixed. e.g. Papa Feral, Megarachnoid (Spider Boss), Gun-Dolph.
Fixed the Casino Courier Side Missions 1 - 5 where you could sometimes be unable to proceed due to missing NPCs.
Fixed the custom camera for ‘Dark Horse’ Side Mission.
Fixed the custom camera for Bartender in Stimtown.
Fixed the situation where sometimes ‘Dark Horse’ could not be completed due to the NPC remaining in a frightened state.
Fixed an issue where the Volatile Augmentation when used on neutrals had no effect.
Fixed an issue where the Siege Mech in the Prison could despawn after a Player died.
Fixed the issue where the Snooze Dealer’s Journal map icon was a white square.
Fixed damage scaling on the Pocket Mech minigun to make it more powerful as you level up.
Allows the use of certain augmentations while rolling.
Balanced cost/cooldown/efficiency of Life Transfer, Neutron Beam and Overclock
Fixed issues with PLUG Cyberdeck not cracking ICE 1.
Translation (all platforms)
Lots of fixes have been made to address instances where English would appear instead of the correct language.
Further work is continuing to improve the quality of the translations as well as further general bug fixes.
Fixed an issue in Japanese where text overlapped in the intro cut-scene.

Fixed an issue where sometimes in Dreamworld, shooting SFX without enemies being present could be heard.

I did note there is a named mob "Chunk" that can be found in "Chunk's Basement' in the middle of Black Lake Towers Zone. I have killed him a couple times on the latest patch and he is not added to the enemy list.
Also finishing the game again on the latest patch and it did not add 'Dakyne' to my list.
Hope a fix is in the works, seeing the 0,01% stat.
Comprehensive achievement is still not unlocked after getting Flatliner. My codex matches the authors list, with the addition of Interdimensional Calamity.

Yes, this was on steam for me.
I played all the game alone.
The best the studio could do is launch an advanced game keeping the level + weapons + armor + money to be able to make the missing achievements

I hope that all missing entries can be added retroactively with a future patch in a similar manner, but obviously I don't know if and when this will be possible.
Because we still have to wait for the fix for Comprehensive, I would wait until at least that patch before starting a new playthrough just for this achievement. I didn't even expect that Flatliner would be fixed by this patch in the first place, as their Twitter account said the achievements would be fixed by the next one. So, maybe then all other missing Codex entries can be added. 🤞

I just killed a second just now at silo 86, but he doesn't want to appear in the list. My Flatliner achev is block at 96 % =/

edit : I got it ! I played the game on the PC GamePass, uploaded the save, then ran into a Siege Mech in The Glut area, and it popped.
Reminds me of Forza 7 with achievments stuck on One X, but popping on the GamePass PC...
Another place you can try is the very end of the Silo 86 area in deepStink, near the entrance to MenShen.
Could you update me if you find one in either place or not? I am currently writing the walkthrough for this game and it would be nice to include alternative enemy locations.
It's funny because now I have Comprehensive unlocked and Flatliner still missing lol

Even got the "comprehensive" achievement halfway trough the game so i was suprised that this bugged for me, lets hope future patches fix it.
The fight did end when I killed both his guards though so maybe something to do with that?
Fresh run, all solo. Will wait for a fix.
Went through every single location on Hightierstreet there wasn't a single nug engineer...just a few nug gadgeteers by thoms gate upwards to the cosmodrome..
So I went to the Warrens and visited "The Edge"...and guess what Nug Engineers were there...
just leaving the info if anyone else is missing these..
@PheasantFlucker You should be able to find it in Scrapland. If it doesn't spawn for you, you can try to travel between areas or reload the game. Hopefully, that will help you to finally get it. Good luck!
I am trying this by exploring all over again. I will keep you updated but it won't be quick as I have my kids the next few days and won't be able to game much.
Nevertheless, I recommend waiting at least for the next patch, which may fix these missing entries as well as the ones for Comprehensive.
On my Xbox I logged in resumed saved game and the achievement popped and the new codex entry (Interdimensional Calamity*) was added successfully.
Anyway, thanks for letting us know that the achievement is still buggy, even with a new character after the latest patch. I have no idea what's causing these issues for so many players. I just hope that with the next patch it's finally fixed for everyone. In the meantime, I flagged the achievement as buggy.
BTW, was this a solo playthrough, and which version(s) (i.e. patch) did you play?
If this is your last achievement, you could speedrun through the main story on easy and do only the side missions with unique enemies (I think only the Scientists in the side mission Bubble Trouble but I am not 100% sure). Though this is not ideal, it should be a lot faster than your first full playthrough at least.
The downside to this is that both characters are on the same screen and one cannot run forward without the other following. It takes 2 people or you could play single player and go to coop couch each time you get in a pinch to bring in the big guns.
@xxDAMRONxx Strange that the achievement didn't unlock. I don't know if you tried this already or if it will work, but you can try to fix it by switching between characters (make a new one if necessary) and loading up your save again after switching back to your main character. 🤞
