update October 18, 2021: Patch 3 (version 4.2.4.0) has been released on Xbox. While the patch notes mention a potential fix for this achievement, I am unsure if this will fix all known issues with it (let us know in the comments).Previous updates are under here for brevity
update August 19, 2021: A patch (version 2.1.4.0) has been released which I assume coincides with a previous Steam update, and should fix some bugs related to the side missions and saving progress while playing in co-op. Hopefully, everyone can now complete all side missions and unlock this achievement. Unfortunately, I can't test this so let me know if this patch fixes any of the issues you were having before*.
* As mentioned in the comments (thanks) the patch will not fix this achievement if the progress bar was stuck prior to this patch and requires a new (partial) playthrough.
update August 12, 2021: I get many questions and comments about the patch, so I thought I would give some extra info. As of now, the latest patch for the game is not yet published (current version: 2.0.5.0) as it still needs to be approved by Microsoft (source). We won't know for certain which of the issues will be fixed by this patch until it is live (Steam patch notes), but it seems like that could be very soon. I will update the solution when I know more.
Side missions
In total, you need to complete 28 side missions, listed alphabetically below, so you can check which ones you still need to unlock the achievement. Most are easy to find on your map using the yellow rhombus (or diamond), however, the multiple variants of the Casino Courier missions (5 in total) in the Golden Satori (center of the Arcology: highStreet) can be easily missed.Some of these side missions become available by progressing the main storyline or completing certain side missions. Fortunately, it is possible to finish any of the side missions after the main missions are completed. I think the side missions themselves are easy to complete (follow the marker and do the thing, repeat...), so I have not included walkthroughs here.

Bugs & fixes
This achievement appears to be buggy for some players (thanks to all the comments) where your progress can suddenly stop. This is most commonly (but not exclusively) caused by switching between characters or playing co-op. Therefore, I highly recommend doing a full SOLO playthrough with a single character.Under here I will list potential bug fixes. Let me know if they worked for you or if there are more.
If your progress was stuck despite having completed all side missions prior to patch 3, you can potentially unlock the achievement by switching between characters on the main menu - create a second character if you don't have one already - and then load up the save in which all side missions are completed (thanks vickybuddie).
Missing side mission in Log
It is possible to have a side mission missing from the mission log even after completing it, making it impossible to unlock the achievement in that playthrough. However, some players report that they've unlocked the achievement by completing that missing side mission in a second playthrough. They did not need to complete all side missions again in that second playthrough.
After completing the missing side mission, you might have to switch back to your original character (i.e. the one with the missing side mission) and load up the save to unlock the achievement as noted by TB4CO in the comments.
The Champ
If you experience a bug during the side mission The Champ, where you cannot progress "Survive the Carnage", you can try the following fix provided by Hybridchld and Fejer: Set the quest to active, go to the area behind the arena on the other side of the bridge, and get killed by the enemies there. When you respawn after death, the ferals from the side mission should also respawn, making it possible to kill them and progress the mission. Fejer also mentioned that he was careful to kill the ferals without using tactical abilities.

Edit: The couch coop workaround didn’t work. But the online coop way worked. You have to have a friend or a 2nd account, which needs to create a new game. And then get invited via online coop to that new game. There you need to complete the missing side mission with your glitched out character and achievement should pop. Luckily, my mission was right at the beginning.
I've read on the patch notes (patch #4 on Steam) that the devs are working on multiple fixes that are introduced during the latest patch but they don't specify which issues.
I did make sure to go complete the main story line before starting any side quests for I think that main line progression is involved in the side quests being bugged out due to dependencies on access to areas opened up by mainline quests being a pre-requisite even though you can obtain start and try to complete said side missions before you have completed the main line pre-requisites.
Note: Starting a new game or joining a new game that is not past character generation stage with a currently saved character slot will wipe all character data. The game warns you if you are about to do this and tells you it will overwrite data. If you see this do not move forward.
Game Progress Reset Process
Sign in to xbox with two accounts (One can be a guest).
Load Ascent with your main account the one that has the character profile you wish to reset game progress on.
Start a new couch co-op game with a second character save slot.
Then start the game so it gets past character generation and gets in to the first mission allow it to auto save then back out.
Resume the co-op couch game with that same character using the main account.
Once in lobby go to one of the available slots with the second controller/account and press A. This will bring up the list of saved character slots of the main account.
Select the main character that you wish to reset game progress on then start the couch game again allow it to auto save then back out of the game.
Now you will notice the character profile has been reset to the start of the game.
Simply go to single player and resume game under the main profile you just reset and away you go with full character data.
As for the "For both our benefits", it's not even mentioned in the patch notes (https://www.curve-digital.com/en-us/ascent-patchnotes). Still, I had 23 missions out of 28 done, the achievement tracker was at 50% before the patch and stayed at 50% after the patch.
The thing is though, technically, it's totally not a problem to fix any of the achievements retroactively - I mean, they are clearly tracked as part of the save game - I can see that I have max level weapon, I can see how many side missions are done, basically, the fix should be about reading these and triggering the achievement action. Plenty of games I played did that sort of fixing. In fact, a lot of games just do this sort of thing by default - every time you load a game, they check the save game stats and trigger the achievements if necessary.
- leave the area (walking/fast-travel/switch tier) and return. There might be some enemies that are missing and the game will think they are still alive.
- die and respawn
- the classic: turn the game/xbox off and on.

@WB ETERNAL Not sure, but it could be in Blossom Hills. It's one of my last ones, so it could also be that you need to complete other missions to unlock this. Can you (or anyone else) update me when you find out?
"Dreaming High" was the only one I was missing as well. But I just got it. As soon as I finished and turned in the quest for "Dreadful Situation" to the dude at the bottom right corner of Blossom Hill inside the Litvinov Cab Company, he gave me the quest for "Dreaming High"

I started a co-op game with that save and this time it allowed me to progress; not that it means much as my counter for side missions is stuck at 53% anyway! haha.
Also finished all 28 and this didn't pop for me

Completed Silicon Optics but it disappeared from my list of completed side quests. Couldn't re-trigger it.

i've done the 28 missions, dark horse inclueded, but i can't see it in my done quests, it just dissapeared.

Anyway, amazing game, I really loved it!
Today, I started a new character, did a sidequest and my % went to 96%. So I did a second sidequest and the achievement popped.
So I now have all 28 done on my main character but only 64 complete according to the tracker!

Got a problem here: Completed all side missions, but there are only 27 on my list. "You snooze you lose" is missing, even though I checked if I completed it. It just doesn't show up. Anyone can help?
Probably I never talked with Lockesley because he is not in my codex, but I'm not sure.
So he probably is somewhere else now...
I don't think the starting points of the side missions should change. At least, I haven't encountered that so far in my playthroughs.
I refuse to play the game again just for this, so they better fucking fix it.

If these things don't help you, then I am out of ideas, unfortunately. I tried to search for a fix online, but I found nothing so far. If all things fail, you can submit a bug report and hope the devs will fix it in a future patch.
@Zoppa860 That sucks. I too had some problems with a couple of side missions (not this one though) during my playthroughs. Lucky for me, restarting the game always worked in those instances. Not sure what else you can do to fix it 😓

I know this doesn't help anyone, but just because the game is patched doesn't make all of these glitched achievements appear.... not sure if I want to run through the entire game again... ugh... that's a lot of running and fast traveling.

I'll have to give it a go on a second play through after I complete the story.
EDIT - Yup, had to complete all side missions on my second playthrough and thankfully none disappeared. Guess I got lucky on this save.
Edit: Beat the main game, currently Level 36, nearly done with the DLC and they're still not appearing.
I am 100% sure that I completed the humidity doctor. I am even using the passive augmentation you get as a reward.
But the quest is not even listed in my diary. I just don't see it in the history of completed missions at all.
Right now I'm stuck at 71% and I don't see any other side missions anywhere.
Okay, I've found "The Champ" and "That Guy" it just wasn't visible on the map.
But I'm still missing the Humidity Doctor which I know for a fact I've already completed, I remember the mission very well. But it's not listed in my "diary" at all.
I'm stuck at 64% and I finished 'Casino Courier V' but it's the only mission not listed in the codex.
EDIT So I played 3 playthroughs. If you switch platforms (i.e. Xbox >PC) Your achievement progress stops. I had a solo playthrough that was tracking achievements on my Xbox one S and as soon as I loaded the profile from the cloud on my PC and played with a different character my achievement progress stopped.

Can anyone else confirm that if you start another playthrough after a stuck progress bar, that you don't have to do all 28 side missions, but that the progress increases with each completed side mission?
Are you seriously saying I I’d have to replay the entire ****ing game including all side quests now, or should it be enough to replay only the ones that haven’t counted? Maybe there’s a way to find out which ones didn’t count?
Also, I’ve saved the final quest in case it’ll be needed later to trigger the achievement. Is there any ****ing hope it could if they patch this mother****ing ****?
Now, you can start this side mission during the main mission Trace Protocol. I started it after visiting Blossom Hills, but it might be possible to start it sooner. You may need to fast-travel to get Lockesley to appear, though.
I hope this helps.
Basically, you can start a new game and the achievement should unlock when you complete the side mission you were missing (in your case Balls Deep); you don't have to redo them all. Fortunately, this is one of the earlier side missions, so it shouldn't take that long to complete.
Of course, you can first check Serenity Plus in Cluster 13 if you somehow missed it.
Its fixed now if you stand in front of the guy and kill enemies there it glitches the enemies out, I had move down towards the entrance for it to work. Thanks for the help.

There are 28 missions. 5 of them are reoccurring as in you must visit the NPC a total of 5 times to do similar quests. The quest in question is the " Casino Courier 1-5". The NPC is located in the Golden Satori Casino in the center of High Street near the elevators. You must go back and forth to complete and pick up the next part of his quests.
If you have already done the first part you should be at 85%. Each additional part of the quest you complete will be around 3-4% that will get added. Example: Beating the Casino Courier 2 will net you at 89% if you have already finished the other side missions. I suggest waiting until you unlock the Pinnacle (the highest part of the tower) before doing Casino Courier 4 since it takes place in that floor and cant be completed until its open. Upon completing his 5th quest you should unlock the achievement. This may or may not work if you have changed characters or joined a coop session. Hope this helps.

The marker in the "Stash" task does not point to the right place: you need to go down to the pizzeria along the stairs on the left.
