This has to be done on at least Hard. I tried this on Medium and scoring 100% I only got a 343-note streak.

I did this song on hard and I missed a note right before the outro ( when they repeat My Baby Dont Care) and I got the achievement. Also I tried on medium and tthere wasn't enogh notes.

Just do as it says. This can be done on Hard or Expert mode. When doing this, watch for the tempo changes. Sometimes you will need to strum at a steady, fast pace and it will slow down for a couple notes before it starts back up on that steady pace again. It can be pretty tricky but after a few practice runs or just listening closely to the song, you will get it. Here is a video of the song chart on Expert guitar.
