Strange. I definitely did 3 in my run but did not get the achievement.
I did two deals in one room and one deal in another.
Notably, I was playing in coop. I'm not sure that would make any difference.

To unlock Azazel You must make 3 deals with the devil in one run. Your best chance in making a deal with the devil is to not take any red heart damage on the floor your on but it does not always happen. *To ensure a deal with the devil you must get an Item called Goat's head, you will get a deal with the devil after every boss fight.*
