How to unlock the Bethany achievement in The Binding of Isaac: Rebirth - Definitive Guide

This achievement is more or less self explanatory, but I would like to add, as well as it being Hard Mode and not dying/ losing any lives, that this unlocked for me after defeating It Lives, as that is technically the earliest point your run could end with a win if you dont take The Polaroid or The Negative, so dont worry if you think you have to get to any further floors, killing Moms Heart/ It Lives is the cutoff point for this! smile

Weird, for me I didn't take Polaroid or the Negative. Beat the heart then had to take either to Satan or isaac as no chest appeared. Beat isaac and jumped in the chest. No cheev. So I guess maybe it's glitched or I needed to go further.
