How to unlock the Dead Boy achievement in The Binding of Isaac: Rebirth - Definitive Guide
This has been patched. Maybe you can still download this game without DLC and without applying the update.
Going for the no damage achievements is actually very easy using an exploit:
Everytime you back out to the main menu you get an autosave. So keep that in mind.
After you've cleared one or more (how many you like) rooms without being damaged exit to the main menu to get another autosave.
In case you get hit you have to either let enemies, spikes or bonfires kill you or bomb yourself to death. This can be a tad annoying if you have 12 hearts towards the last couple of floors like the "Womb/Utero" or the Darkroom/Chest.
NOW THE IMPORTANT PART!!!
During the death animation pause the game and hit the guide button. Manually quit the game.
VERY IMPORTANT!!!
You MUSTN'T quit after being hit BEFORE you die. That way it would for whatever reason save your current health.
VERY IMPORTANT!!!
If you continue after starting the game up again you will be back where your last autosave was.
This works for THE LOST playthroughs as well!!!
Of course with him killing yourself is easier since he has no health and dies in one hit.
Items like Holy Mantle (you can take one hit every room), the "9 lives cat head" or guppy's collar (1 extra life) can change this, though.
Avoid the Ankh at all cost since it respawns you as another character and would therefor void the achievement. Same goes for Lazarus' Rags (return as revived Lazurus upon Death) and Judas' Shadow (return as Dar Judas).

So just pray to the RNG God to be lucky with drops and this is going to be a breeze. Time consuming but easy.

One of the easiest (though the most time consuming) ways to do this is just to get the Gnawed Leaf and any familiar that attacks on its own in the same run. I, for instance, had both Robo-Baby 2.0 and Gemini when I earned this, but many others would work as well. The meat/bandages familiars, the leech, BBF, Demon Baby, Peeper, Sissy Longlegs (or the Daddy Longlegs item), Mom's Knife, etc should all work as well.
Just enter the room, try to position yourself close to the center if you have the time, and wait it out. Rinse and repeat with each room until you've cleared the boss.

I got pretty lucky when I got that achievement. I was playing Azazel and that was the only floor I didn’t take damage on. I had Mutant Spider and 20/20 which game me a lot of AoE with a lot of tear and damage increases as well as some slow effects that made dodging really easy. I wasn’t expecting much from that run since I went in with the goal of taking on Mega Satan before I realized I didn’t have the key once I got there. So I guess task failed successfully?

One of the easier ways to unlock this achievement without exploiting would be to use an emperor card as soon as you get into the chest and then not taking damage to Blue Baby (???). Holy Mantle will help because it allows you to take a hit without taking damage every room. Shielded tears could also help block bullets, and any orbitals that block bullets could help out too. Another way is to just play the game normally and maybe you'll get without trying.
See "Basement Boy" for more info. In order to unlock The Chest/Dark Room you must beat Sheol/Cathedral five times respectively to get "The Polaroid" or "The Negative."
Depending on which one you take after killing Mom and beating Sheol/Cathedral you will be taken to The Chest/Dark Room. "The Polaroid" takes you to The Chest, "The Negative" takes you to The Dark Room.
