I earned this with a Devil Item. It's called "We need to go deeper!" I started a new level after going through the trap door. Once on the next level, I activated it. Easy peasy when RNJesus blesses Issac.
Complete any two consecutive floors without taking red heart damage, I'm still trying to find out if blue, white or black heart damage counts. Edit: They do count so don't get hit.

It's probably easiest just to beat the first two levels without taking damage.
Be aware: if you make a deal with the Devil, the trophy might not unlock. I did so and unlocked the secret but not the trophy itself. I needed to delete my save after uploading and then re-downloading it to make it pop. However, by then I had beaten two levels in a row again without taking damage, and without making a deal with the devil.

Easiest done on the Basement I & II. If you get lucky and get an insane early item such as Polyphemus this will be a cake walk. Taking any sort of damage will void the achievement including Soul Hearts and Black Hearts. This is also possible on later floors if your character is overpowered and have the Holy Mantle as it will allow you to take a free hit every room.

The simplest thing is to quickly fly through the first 2 levels from the start, while the bosses are weak and the monsters are not bothering you. A little experience and you will pass without problems.
