The Forgotten is unlocked by doing the following
Kill the boss on floor one within 1 minute of your run starting. If successful you should hear Mom call out Isaac's name. Return to the starting room of the floor and use a bomb which will cause a broken shovel handle to appear in the centre of the room.
Picking up the shovel handle will cause Mom to follow you around and occasionally try to stomp on you. Using the broken shovel will stomp Mom trying to stomp you for the current room or wave if you are in a mob/boss traproom.Pyrokinesis and Host Hat are great items for this as they negate the damage from the foot.
You must carry the broken shovel all the way down to Necropolis II, during that time you can't pick up any other active items or you will ruin the run.
Once you reach Necropolis II kill Mom (you won't have 2 feet trying to kill you at once). When Mom is dead only The Negative will drop and the door to Boss Rush will be open, which must be completed to go forward.
Upon completing Boss Rush the second piece of the shovel will be awarded (Note - You cannot pick up another active item to use during Boss Rush then collect both halves. The second half will only drop if you have the first half on you). Once the shovel is complete you no longer have to avoid Mom's foot.
With the shovel complete it will work just like the item We Need to Go Deeper. Holding the shovel fight your way down to The Dark Room where you need to look for a room with a visible shallow grave in the middle of it. Stand on top of the grave and use Mom's Shovel and you will have unlocked The Forgotten.

So this achievement may be glitched to some extent. I have unlocked The Forgotten, even done a few runs as them, and yet the achievement has not unlocked. Hopefully this unlocks eventually/gets fixed because beyond finishing off 1000000% I have every other achievement =/ EDIT: It just unlocked as I opened the game again thankfully, over 24 hours late but better than never!

Host hat does not negate damage from the foot anymore. Just found that out the hard way!

To attempt this you must first have beaten The Lamb in the Dark Room.
Start a normal run as any character you're most comfortable with. I'd recommend Azazel just because flying and Brimstone can make it much quicker. You need to beat the first level boss within 1 minute of starting. If you pull it off, you'll hear Mom yell out and stomp across the floor. Go back to the starting room and there will be a large shadow of a shovel across the floor. Drop a bomb anywhere in the room, and the Broken Shovel will appear. It is an active spacebar item you must carry for the rest of the run. While holding it, Mom will constantly stomp down on you with her feet like she does in her boss fight, so you have to constantly be on the move. This can make it hard to use things like slot machines or beggars as she will usually destroy them, since her stomps count as explosions. Activating the Broken Shovel will make her quit stomping for the current room, handy for boss fights or other hectic moments. Also since the stomps are explosive damage, if you can find Pyromaniac or Host Hat, they won't hurt you anymore. You can swap the Broken Shovel for another active item temporarily, but if you leave the room without it, it will disappear.
When fighting Mom, the stomps from the Broken Shovel will stop and the fight will be as normal. Afterwards only the Negative will appear, Boss Rush will open, and the entrance to the Womb will be closed. You must then beat Boss Rush, and you must be holding the Broken Shovel at the end. If you do so, the other piece of the shovel will spawn as a reward at the end. Take it, and it will become Mom's Shovel, and the stomping will stop for good. Mom's Shovel functions identically to "We Need To Go Deeper!", letting you dig holes either to the next chapter or to crawlspaces. Grab the Negative if you haven't already, and head on to the next chapter.
Continue on as normal all the way to the Dark Room. Here, you are looking for a special room with a grave in it. It stands out fairly obviously, a weird square raised patch of dirt usually right in the middle of the room. Stand on the patch and activate Mom's Shovel. You will dig up a skeleton and a ghost will fly off the top of the screen. Don't leave the room too early or it might not unlock.

Yo Nice Guide, I Just Wanted To Add Something For You
- If You Don't Own Repentance Yet, "Host Hat" And "Pyromaniac" Still Negate Shockwave And Stomp Damage. So Pressing The R Key On Floor 1 Until You Find Either Of Those, Makes The Entire Run Waaaaaaay Easier.
I Could Not Do It Without The Host Hat I Got, It Was So Hard.
