The Binding of Isaac: Rebirth

If you have Magdalene unlocked (requires you to have at least 7 red hearts), she makes this achievement rather easy, as she starts with 4 red hearts and an item, the "yum heart" that gives you a full heart with every use, and doesn't void the achievement.

Just start a run with Magdalene and make sure that you don't pick any kind of heart (red, blue, black or white) during the first 2 floors. This will unlock at the start of the third.

To unlock Eve you need to beat any 2 levels in a row without picking up any type of heart pickup, this includes Regular Hearts (Red), Soul Hearts (Blue), Eternal Hearts (White), and Evil Hearts (Black). You can however pickup items that give you more health and it will not void the achievement, Also when you have done the two levels the achievement will not pop until you reach the third level, so don't worry if you kill the boss and nothing popped.
