The Binding of Isaac: Rebirth

NOTE: This guide was made before Afterbirth was announced for consoles. The achievement itself is exactly the same once you get Afterbirth, but there are some changes. The Lost's unlocks can be a bit easier to unlock once you earn the ability to start with Holy Mantle, and Meat Boy is easier to get with Lilith's starting item. Also, this achievement is only for getting 100% of the Rebirth items, any new items in Afterbirth count towards the 1001% achievement instead. This can make it tough when lots of new items show up when you only want to find the old ones.

100%ing the game consists of 2 parts: seeing every ending and picking up every item. Your ultimate goal is to completely fill out the items, endings, and secrets pages under "Stats" on the main menu.

There's no real going out of your way for the endings, you'll get them as you play through the game, except for one that takes a little more effort.


Now for getting all the items in the game. First, you have to unlock them all.

Aside from that, you will need to beat every boss and clear boss rush with every character. You can easily track your progress on the character select screen. On the top left, you will see this box:
The symbols appear as you beat the corresponding boss with a character:

Only The Lost has to beat all bosses on Hard Mode, everyone else only needs to beat Mom's Heart/It Lives on hard to unlock a co-op baby for each. (do not even attempt to play as The Lost on Normal Mode as it will not unlock anything) (see uhKillswitch's solution for some help with The Lost)

So, now you've manged to unlock all the items in the game. Now what?
Picking them up of course! Head on over to the Items page under Stats on the main menu to see everything you've picked up at some point. Anywhere you see a question mark, you haven't found that item yet. You will also need to become both of the game's transformations at some point.

You will ALSO need to create level 4 Meat Boy and Bandage Girl. These are probably the toughest and most annoying things to get. Tips here: http://bindingofisaacrebirth.gamepedia.com/Cube_Of_Meat (works the same for Ball of Bandages, Famine will randomly drop either Cube of Meat or Ball of Bandages, and the rest of the horsemen will also drop the same for the rest of the run (except Conquest because he is a jerk)) Getting a level 4 Bandage Girl also unlocks The Bandage.


It is quite a grind, and not for the faint of heart. I have Real Platinum God on the Steam version, and I hope to unlock it on Xbox One before Afterbirth comes out and makes it even tougher with more items to juggle around.

This is NO 100% guide. This guide explains how to exploit the save mechanism to avoid taking damage/dying playing as the Lost. Since you need to beat the game two times on hard with the Lost including the Boss Rush Room (you only need to do this 1 time; correct me if I'm wrong). You need to kill ??? (Blue Baby) which you do by taking the Polaroid and going into the light after fighting Mom in level 6 and killing the Lamb which you do by taking the Negative and going into to Devil Door.
Now for the exploit:

Everytime you back out to the main menu you get an autosave. So keep that in mind.
After you've cleared one or more (how many you like) rooms without being damaged exit to the main menu to get another autosave.
In case you get hit you have to either let enemies, spikes or bonfires kill you or bomb yourself to death. This can be a tad annoying if you have 12 hearts towards the last couple of floors like the "Womb/Utero" or the Darkroom/Chest.

NOW THE IMPORTANT PART!!!
During the death animation pause the game and hit the guide button. Manually quit the game.

VERY IMPORTANT!!!
You MUSTN'T quit after being hit BEFORE you die. That way it would for whatever reason save your current health.
VERY IMPORTANT!!!

If you continue after starting the game up again you will be back where your last autosave was.

This works for THE LOST playthroughs as well!!!
Of course with him killing yourself is easier since he has no health and dies in one hit.
Items like Holy Mantle (you can take one hit every room), the "9 lives cat head" or guppy's collar (1 extra life) can change this, though.
Avoid the Ankh at all cost since it respawns you as another character and would therefor void the achievement.

So just pray to the RNG God to be lucky with drops and this is going to be a breeze. Time consuming but easy.

This is NOT a 100% guide, but guide on how to get the Bandage Girl/Super Meat Boy secrets, the hardest ones to unlock.

Getting these two secrets are by far the most annoying, since you have such a low chance of getting it, and not only that, you have to get it twice.

To get Meat Boy you need to get the "Cube of Meat" item four times in one run, whitch means fighting Famine, Pestilence, War, and Death in the same run, same goes with Bandage Girl but with the "Ball of Bandages" item instead.

The most common strategy for this is to keep resetting your run until you get the "Book of Revelations" item on the first floor via item room or library. Next you need to activate the item at least once on the first floor of each chapter (Basement, Depths, etc.), things that may stand in your way are double boss rooms, Mr. Fred, and Conquest. Birds will be flipped. If/When the chances of getting Meat Boy/Bandage Girl will be very low.

Overall this grind will take many hours, especially on consoles since (for some reason) there isnt a restart button and instead you'll have to return to the main menu evey time you dont get the book of revelations, just remember, many other people are feeling the same pain as you.
