Mom's Heart is the final boss in Utero II.
To get to Utero you first need to have defeated Issac's Mom and got that ending.
I recommend using Azazel simply because his attack is very powerful.
Kill the heart three times and pop! You've done it.

This will unlock through natural play. After defeating Mom once you will unlock The Womb. Simply advance through The Womb I & II and defeat Mom's Heart three times.
