How to unlock the Magdalene achievement in The Binding of Isaac: Repentance - Definitive Guide
You need to have 7 red hearts at once. This can pretty simply be done in any mode, but I would recommend Greed mode.
In Greed mode, each floor has a silver item room to access for free which contains boss items. This means you have a decent chance of getting a health up on each floor real quick.
The thing that makes Greed mode good is the shop above the main room. Health very commonly appears for purchase in there.
You gain coins by defeating waves which you start by pressing the button in the main room. You should make enough to buy at least 1 item per floor.
With some luck and a couple restarts to get a good first floor, you should be able to get this done really fast, as soon as two floors in.
(Just don't take Deal with the Devils and sacrifice your all-important hearts)
ALTERNATIVELY:
With Repentance, they added couch co-op that still allows you to unlock achievements. When you play co-op, bosses drop 2 items. So, you can plug in a 2nd controller and start. Since boss items are commonly health, you have a very good chance of getting up to 7 hearts very quickly. Just pick up all upgrades with 1 character. (Don't worry about the 2nd player dying, the run continues with no punishment)
