Sorry ... it's been a while and I don't recall the specifics but I really thought it was story-related. That said, I've had a few achievements that don't pop when they should and giving my console a hard restart and re-launching the game has _sometimes_ helped.

Didn't pop for me. Even after hard resetting and speaking to the book again. This is after you make the medicine drink and then speak to the book to confirm the surprise but it didn't pop for me
