This is easiest done on Floor 1, levels 1-5 (the first 5 levels). You can get hit, use powerups, and use perks, you just can’t die. Play on Easy so you have more Health and also so there are fewer enemies. Once you finish the 5th level without dying, the trophy will unlock.
