In the wave 50 solution (Big Game Hunter), the creator and some other players mention that activating the boosts at the start of a run is preventing achievements from unlocking. This didn't happen to me but I guess it could for you.
It's simple, just don't purchase anything for 5 waves, until the achievement pops.
It's easiest done for the first 5 waves but can be done at any point of a run providing it's 5 consecutive waves. It's very easy to do at any point of a run and doesn't really require any planning. If you're doing it later in a run, just make sure you've got some good upgrades, a few health injectors, and a good amount of ammo just in case things get spicy.

I was able to obtain it with Boosts on and within the first 5 waves, I just had to turn off my game and restart it. Don't know what the reasoning behind it is, but I wasn't able to obtain it before doing this, and I made it to wave 11 twice without a single purchase.

There is still a glitch if you pop rampage mode that will lock you out of this. I popped it today making sure I didn't use rampage mode.
