How to unlock the Grim Reaper achievement in The Callisto Protocol - Definitive Guide

EDIT: As of Patch v5.02 (June 29, 2023) - 11 of the 43 collectibles have been moved as follows:

OUTBREAK
- Moved one log pickup to the end of the hallway before the medical room, after the beta path door
- Moved one log pickup inside the cafeteria section

AFTERMATH
- Moved one corpse harvest/pickup to the corner of the red room

HABITAT
- Moved one log pickup back to its original location in the upper corner of a room onto a box with a valve wheel

LOST
- Moved one corpse harvest/pickup to the far corner of the garage

BELOW
- Moved one log pickup to the left of the moving platform exit

COLONY
- Moved one corpse harvest/pickup to the top of a ladder before the shimmy
- Moved one log pickup closer to a REFORGE station

TOWER
- Moved one log pickup to be inside a prison cell
- Moved one log pickup to be inside a Biophage infested hallway
- Moved one log pickup onto a table by a cherry tree

The Callisto Protocol has 43 Bios collectibles found throughout the 8 chapters of the game.

This game does not have Chapter Select or New Game +, which means all collectibles in this game are 100% missable. If you miss something, you'll need to either go back using an auto save or manual save, or replay the entire game a second time.

There is also one noted collectible as glitchy, and it should be checked on a few times during chapter 4 to make sure it counts.

Wanted to do a blind playthrough to enjoy myself and missed a handful through natural exploration. I have zero desire to replay the game again without the upgrades I purchased, so I'll wait till NG+ gets added to go back and finish this out. Will be using the guide when that time arrives, so thanks well in advance!

It may be worth pointing out that in Chapter 4 there is a collectible that can completely wipe if you die after collecting it, so you need to make a manual save before collecting it. It's for Sgt. It happened to me and I didn't realize until I was at the very end of the game and the achievement didn't unlock. So frustrating.

Achievement doesn't glitch. The achievement is for all implant bios, not necessarily all sound files. I missed some sound files on my first playthrough, but I got all implant bios. Collectible #42 is the last implant bio. #43 is a sound file.

I 1000% missed at least two of them but still got it. It's still a bit unclear what it is, but I think Ryamus is right.

I think you have to unlock every category in the bios menu. Basically make sure that every person's name is clickable.

Evidently it is about the peoples' bios (key icon 1/1). Those can be unlocked via picking up Audio Logs or harvesting implants for the first time per subject. Once either are accomplished, relative Data-Bios are unlocked. There are Data-Bios for subjects that have only one data available, to be collected through picking up Audio Logs or harvesting implants, but their Data-Bios' entries are receiving full status (key icon 1/1 with waveform icon 1/1). Conclusively, not all data is necessary to be collected, relatively.

Got this on my 2nd play through, with the secret rooms but missing 2 of Mahler’s and and 1 of Cole’s. I did listen to most of them but some I cut off early as I’d heard them on my first run.

I've had item 20 disappear on two play throughs without dying after getting the item. it would vanish after a few check points for some reason.

Just a tip, with New Game + the achievements pops immediatly after picking up any data bios that you missed. I missed one in Chapter 2 and unlocked as soon as I grabbed it.

Are you gonna do a guide for The Commonality? I just unlocked it, and I feel a video guide would be best for it since I believe you have to go to two secret locations for it to unlock.

Be advised, to understand its save game system: Only the status at incremental checkpoints are mapped towards save game data. That means, even after a manual progression save, its save game data is relative to the last checkpoint's status. Not being aware of that, can eventually translate to unconscious negative events, in worst case scenarios. Focused example: Checkpoint triggered, data was collected, manual save performed, manual save game data loaded (after failure; Load Game/Manual Save #; Restart Checkpoint; Continue). That data is not collected on continued progression like that. Focus on saving status for good by triggering a next checkpoint.

So the embedded video is no longer reliable, and the new text explains a lot of them have been moved since the video was made? Why would the game do that? That sucks.

UPDATE: I found the answer on the official website...
ANSWER,
"In order to maintain The Callisto Protocol's narrative structure and avoid over-lapping audio, some of the in-game audio log locations were moved."

I'm about to start this game. Anyone find a new, updated collectible guide? Or is this the best there is still?

SAVE FILE DISAPPEARED on XBox. Solution.

If you turned off, or restarted, your XBox while playing Callisto Protocol, the last save may not appear at the main menu.

It's still there. Don't worry.

At the Main Menu select:
- New Experience
- Skip cutscenes
- Control Jacob in his ship for a few seconds.
- Hit Start.... NOW
- Select LOAD GAME
- Choose a previous save from your last gaming session.

I thought I had lost all progress finding these stupid implant files, plus suffering through fights with no ammo because I sold all ammo to buy upgrades.

Use this same "RELOAD-PREVIOUS-SAVE" technique if you miss a collectible, but your way back gets blocked.

Can confirm with @vK Knave that you only need to collect the collectibles you missed in order to unlock the achievement. However, you still need to figure out what it is you missed in order to get it, game isn’t very good when it comes to the collectible screen.

To add insult to my injury, my save game disappeared when I turned my XBox off and lost all progress.
I was on collectible 37 about to start the final chapter.
The game isn't even that good.
I may not bother restarting.

UPDATE:
You can hit start and choose LOAD GAME from a previous save if your way back gets blocked and you missed a collectible.
A hard lesson I learned the hard way.

Achievement unlocked once I picked up #42 after the turrets were disabled. I missed one collectible earlier too.

CONFIRMED: This guide is updated and the best to follow. I followed the text/picture guide, never watched the video.

To add insult to my injury, my save game disappeared when I turned my XBox off and lost all progress.
I was on collectible 37 about to start the final chapter.
The game isn't even that good.
I may not bother restarting.

UPDATE:
You can hit start and choose LOAD GAME from a previous save if your way back gets blocked and you missed a collectible.
A hard lesson I learned the hard way.

SAVE FILE DISAPPEARED on XBox. Solution.

If you turned off, or restarted, your XBox while playing Callisto Protocol, the last save may not appear at the main menu and you can't select CONTINUE.

It's still there. Don't worry.

At the Main Menu select:
- New Experience
- Skip cutscenes
- Control Jacob in his ship for a few seconds.
- Hit Start.... and NOW you can...
- Select LOAD GAME
- Choose a previous save from your last gaming session.

I thought I had lost all progress finding these stupid implant files, plus suffering through fights with no ammo because I sold all ammo to buy upgrades.

Use this same "RELOAD-PREVIOUS-SAVE" technique if you miss a collectible, but your way back gets blocked.

Achievement unlocked once I picked up #42 after the turrets were disabled. I missed one collectible earlier too.

CONFIRMED: This guide is updated and the best to follow. I followed the text/picture guide, never watched the video.

This is the guide I used from power pyx, easy to follow and segmented. Jus know there is no chapter select after beating the game to unlock the trophy all items must be collected in one play through. It doesn’t count progress across plays. And you don’t have to actually read anything, just pick up the collectibles.

With new game plus, your statement "all items must be collected in one play through" is no longer accurate. I started new game plus with the guide you posted when I got the 6th Data-bios the trophy unlocked. Worth updating the guide to mention that :)

The game doesn't show you the ones you collected previously in NG+ though, so you will have to keep picking them up in order unless you know what you missed

In Chapter 3, 'Aftermath', you'll be introduced to a form of the game's collectibles, "implant bios." The implant bio is effectively the chip in the back of the neck and there are usually five or six per chapter that you can collect. Some are optional, some are mandatory, and because there's no chapter select or New Game Plus, if you miss one, you have to redo the entire playthrough (or load an old save if you have one and go from there).

Fear not though, folks, if you follow our implant bio collectibles guide, you won't miss anything!

What you missed can be collected in the new game +, at first you can’t see what you collected previously, you need to load the last save and see what you missed, you have to collect them in NG+ and the trophy will appear, the game remembers what you collected in the last game but does not show them

As already mentioned, the trophy is buggy, so also be sure to check that all the entries are in place after death/loading a checkpoint.

At level 4, one of my records disappeared after death, I can’t go back and pick it up again - the item is no longer there, I had to load the last save before getting the item. If something is not collected, you will need to go through everything from the moment you receive the missed item. Items are not transferred to the new game; you cannot replay chapters and collect items.

Correction according to the video guide from the first tip.
Chapter 6, the last log from the list has been moved. The corpse now lies immediately to the left of the exit from the platform where you fought the two-headed one. Personally, I just needed this log for a trophy.

Received this trophy twice on the penultimate implant. Be attentive to those checkpoints where the implant is located, after death you can forget to pick it up.

A strange trophy, I think the audio recordings are not important, since I received the trophy, and the doctor did not have one audio recording. Most likely, only those that are displayed as a key in the log are considered. And in the latest version, on ng+, you can see what records you collected from the last game, but records open by chapter.

The trophy dropped on the penultimate entry.
This indicates that the trophy is bugged or new entries have appeared that do not count towards the trophy. Be careful and collect fresh videos. Since recent patches, the locations of the entries have changed.

I missed two entries on my first playthrough. On my second walk through the guides, I realized that the corpse I needed to take the recording from was not loading. This is the head of the colony - Chigwan Park. Neither the corpse nor the action button appeared on the screen. I walked around the room pressing and gg began to cut the recording from the air, and the trophy fell out.

When I received the first platinum on the PS 5 version, I caught a bug with the plot chip at level 4, took it and, apparently, died and it disappeared, I noticed it at the end of the game, but I beat the game on hard.
You can quickly go through version 2 for PS4, on easy, basically half a day's work, on easy the game is quite simple. As soon as you get the Grim Reaper trophy, save the game manually and load it onto the PS5 version, take the last log - and you have 2 trophies, and considering that the difficulty trophy is also done by loading on 2 versions, you will get 2 platinums without any problems. By the way, I got the trophy for collecting everything 1 log earlier, on the penultimate one.

Due to the extremely crooked checkpoint system, items and magazines picked up after a checkpoint disappear forever after two restarts (that is, they simply will no longer be in the game after a checkpoint restart/death). I advise you to save manually and load from the save, and not restart the checkpoint, so as not to lose uncollected items. My very first log from the ship disappeared like this, I noticed this already 2/3 of the way through the game. Fortunately, the trophy dropped when there were 2 more entries left to collect. Either they fixed this and reduced the number of collections, or this is again a bug. In any case, be very careful and manually save often so as not to lose items during restarts.

Be careful when you explore the ship for the first time, there is a recording of the main character and when you return there, there will be a recording of Max in the opposite room. It is the only one that I missed due to the terribly built graphics and atmosphere
