How to unlock the Lifer achievement in The Callisto Protocol - Definitive Guide
Here is a workaround/fix thanks to: X InFlames80 X.
Update: i just did the same method again after beating NG+ Hardcore and it worked but it didn't work the first time i had to restart my Xbox after that it worked! also make sure you're using the same completed save for step 4 & 6 mentioned above.
NOTE: you need to beat hardcore the first time to unlock hardcore ng+
Not a full on guide, however here are some tips I wish I knew when I first started this mess of a TU.
CarcinogenSDAs maximum security no hit walkthrough is very compatible with hardcore with no noticeable differences as far as enemy routing and placement
The 1st and 4th two-head boss fires can be completely cheesed
Two-Head #1: simply run into him and dodge his attack, then run into him when he’s recovering. Repeat until he falls. The ammo saved from this fight is worth much more than the small money his resell item is worth.
Two-Head #4: shoot the enemy in the cage to the left to make him chase you. Once he’s dead trigger the boss appearance by slightly turning the corner and lure him to the huge fan just next to your side/left. Back him in using the same method as Two-Head #1
On the tram section, stand by the front or the back as the first 3 spawn here. Simply pressing RB if the ability is bought will blast them down the tram. After the mid tram spawns are finished, 3 or 4 of the zombies spawn and climb the trams side bars one after another. RB attack them as they are climbing and exposed enough and this is an instakill. These two techs got me an easy no GRP 7-8 kills consistently.
Holding more GRP battery packs and less healing is very ideal since you rarely get hurt when your able to spam this god weapon
buying the heavy RB attack upgrade for the baton allows for stupidly long combos on enemies that kill them faster than just normally combing them with no upgrades. I would hit RB and wait for both of their feet to be planted while they sway their body like a drunk then hit RB and repeat one or two more times, totaling 2-4 RBs before I normally combo and their hp is usually shattered. Only do this on already mutated enemies as I expect an enemy mutating to regen it’s lost health
the only gun you need to use is the shotgun. I barely found myself using the hand cannon and I never bothered with another weapon.
on the stealth/crouch and stab section I found a lot of the enemies able to despawn by catching their attention, running to checkpoint, and reloading the checkpoint. I didn’t notice this work as well with other segments or chapters.
I walked into the final bossfight with my riot shotgun at 8/150 and I finished it with 8/70 including the second shots fired with the mag upgrade, so it took around 40 shotguns shots and some explosions to kill him

Does the normal NG+ achievement stack with this one?

Wish I'd have known about those couple of Two Head cheeses, would've saved me some ammo. Ah well. Game is easy af anyway.

At the moment, after completing the usual standard mode, you cannot select new game + and select extreme mode. You need to replay the game on extreme and then the corresponding section will appear in NG+. To alleviate this difficulty, I wrote everything in the tooltip for the adjacent trophy.
PS The bug has not been working for a long time, as tvorojock wrote below. So don't try to restart the CT scan at this point. There will be no point. Just waste your time.

Apparently, due to a bug in the game, the third two-headed (server) can be skipped altogether, just go straight to the panel, Jacob will take out the blown fuse and put a new one and the door will open.
In general, an interesting bug occurred in the game: after the death screensaver, the sign below began to light up permanently to skip, thus during the screensavers you press and they are skipped.

In infection and extreme modes (version 1.028 tested), you can keep the fuse for yourself in the episode where you have to hide from the robot (see Chapter 2). We rise from the supply box with the larva, insert the fuse into the door, and open it. After the burning miracle appears, we retreat, pull it out, and the door closes. The miracle will scratch at the door and then disappear. We insert the fuse again, quickly remove it, insert it, remove it and go through, the door does not have time to close.
