So that is why it was happening. I thought it was because I was playing on minimum security. The QTE's in this game were pretty obnoxious so I turned them off a long time ago and forgot all about it.

Can confirm, just change the QTE to Single Press and/or Auto Complete and voila. I didn’t think about doing this until I was at the fan room heading through Fabrication.
