You can check anytime if you have accidentally killed cultists in the pause menu, and if you did you can reload the checkpoint (or the chapter).

I ended up killing a few cultists in that fight and beat the game, the achievement did not unlock for me unfortunately.

Can confirm, if /you/ kill any of the minions in that particular boss fight, they count as a 'Gloom Cultist' kill, which also adds to the overall Cultists killed stat. It is fine if they die via the reflecting ray attack, however.
