An important bit of information that has not been made clear in any of the solutions: this disc contains two separate campaigns and only one collectible achievement applies to each. Cigarettes are found in Butcher Bay while bounty cards are found in Dark Athena.
What follows in the spoiler is the complete list of 62 cigarette packs found in the remastered Butcher Bay campaign, listed in the order you can get/find them. The content is my own work, amalgamated while piecing together two poorly-structured and poorly written guides (that also left out several packs) from Mahalo and IGN.
If you've followed other guides to the letter and did not get this achievement, skip right to Chapter 24/pack #60. Those three packs are the ones you're missing.
CHAPTER 1 - ESCAPE #35, Bloss - At the base of a ladder right at the beginning of the game.
#1, Cool Drool - Right after you commit your first homicide and the game teaches you about hiding bodies in darkness, you'll continue into room with a bunch of stacked crates that you have to jump around on to continue. Go around the crates to the right and follow the obvious path to find the next pack.
#6, Desert Air - After you learn how to disarm, the cigarettes are on the table the guard was standing in front of.
CHAPTER 3 - PRISON YARD
#25, Voodoo - There's a guy named Waman in cell A37. Talk to him and agree to kill Molina. Molina is in cell A38 directly across from Waman, but the door will not open until you advance the story a bit. Return to Waman for your cigarettes after killing Molina.
MAKE SURE YOU SPEAK TO SHABBY AFTER KILLING MOLINA near the entrance you were paraded through while the game's title cards were on screen. Shabby gives you a keycode to access a room containing a pack of cigarettes later.
CHAPTER 4 - AQUILA TERRITORY
#58, Habibs - Inside cell A28 is the cigarettes. It's also the only cell in this area that is open and unmarked.
#59, Just Regular Grass - After killing Rust and getting a cutscene, speak to Moondog in cell A11.
#57, MBryo 69 - After defeating Rust, talk to Blueboy just outside cell A21.
#26, Count's Finest - After killing Rust, speak to Booger in the yard and buy this pack for 20 bucks. He's behind the bars at the third exit to the courtyard.
CHAPTER 5 - INFIRMARY
#12, Gelding Finest - As soon as you enter the infirmary, there is a door immediately on your left. Using the code you got from Shabby earlier, go inside to get the smokes on the chair.
CHAPTER 6 - MAINFRAME
#13, Pale Male Classics - After adding your DNA and getting your first gun, you'll meander through some venting and a ceiling grate. After dropping down into the hallway you'll go into a room with two guys on a catwalk overlooking where you enter the room, and the cigarettes are up on that catwalk.
#27, Beetle Blend - Automatically granted for killing your first mech riot suit thing.
CHAPTER 8 - THE PIT
#15, Mont Noir - After using the first valve (there is only one way out of the first section after you fall in, so don't worry about it) you will go over a catwalk and then into a narrow corridor where some crazy dude is talking about how you're a special lamb. On your left, before getting to where the dude was, is the cigarette pack in a small alcove.
CHAPTER 9 - POPE JOE'S DEN
#16, OP - After meeting Pope Joe, he'll send you on a mission to get his "voice box". Ride the lift back down and turn left, then follow the corridor until you happen across a room full of assholes trying to kill you. Just inside the room, turn to your right and you'll see a nanomed cartridge inside of an alcove, and further inside are the cigarettes.
CHAPTER 10 - DARK TUNNELS
#28, Karavan - After finally getting Riddick's trademark night vision, you'll be set loose in a labyrinthine area and must find a ladder. There is only one ladder to exit this area, and once you find it, if the ladder were northeast, the cigarettes are in the southeast corner.
CHAPTER 11 - SHOWERS
#29, Yups - You'll come across a locker room. The cigarettes are on the right in the fifth locker.
#30, Q - At the end of this chapter, you'll come to a large door marked Hangar A412 which requires a retinal scan to open. Before using the retinal scanner, turn around and you'll see a railing behind you. Jump over it and down into the lower area, then head left through the vent to arrive in an armory with assault rifles, an armored guard uniform, and a pack of cigarettes.
CHAPTER 12 - GUARD QUARTERS
#33, Guards - Buy them from Yu.
#31, Gronkos Bar - Talk to Jenkins to get some booze before using the elevator, then take the elevator up to find Chancellor and agree to sell it to him for 50 bucks. You'll also get a pack of cigarettes.
CHAPTER 14 - TOWER 17
#5, Charlies - After regaining control of Riddick when he's released from his shipping container thing, go out to the right and into the very next container to flip the bunk for this pack.
These three areas are a mission hub and things get complex here. This section of the guide will be more walkthrough than collectible list so you don't miss any of the smokes.
Talk to Centurion and he'll tell you to challenge Harman to a fight. Challenge Harman, talk to Centurion to start the fight, beat Harman's ass, then talk to Centurion again. He'll suggest Baasim, who is found in the Recreation Area.
When you reach the Recreation Area, you'll see a big spire in the middle with an A painted on it. The Recreation Area is split into four sections labeled with letters, and Baasim is found in section B. Talk to Baasim to get your fight. Before leaving, talk to Pink in the same section to buy a pack of cigarettes.
#56, Yoshimi - Buy them from Pink in Section B for 10 bucks.
Continue around the Recreation Area clockwise to get a mission from Jamal Udeen in Section B; Gulag and The Nurse in Section C; then into Section D to get a mission from Twotongue. Then, head back to Section A and buy a shiv from Cricket. This is mandatory because it triggers Asif to head to the secret stash which contains items you need to finish other missions. You'll get this money back later so don't worry about it. Now, head back to Centurion to start the fight with Baasim. Once you beat him, you get a pack of smokes.
#18, Yoyall - Beat Baasim.
Talk to Centurion again and he'll tell you to challenge Sawtooth in the Feed Ward. Head there and speak to Cuellas to accept his mission before continuing on, and you'll get attacked by some asshole who isn't happy that you killed Harman. You have to kill this guy to get the poison you need to complete The Nurse's mission, so make sure you pick that up before continuing.
In the very next room is Binks, the guy you need to kill with the poison. Right next to Sawtooth is the food tray Binks eats from if he's not sitting at it when you arrive. Nobody else here seems to care what you're doing, but make sure Binks isn't watching you when you poison his food. After that, speak to Sawtooth to get your fight challenge, then continue into the next room.
On your left as you enter, there is a very out of place piece of aluminum which is a cache of things you want. Unfortunately, you can't open this thing until Asif comes along and interacts with it, so you'll just have to sit here and stare at it until he shows up. Stealth isn't required -- I was standing on one of the tables when the dude showed up and he didn't seem to care. When Asif finally turns up, wait for him to walk away again and loot the stash.
Gomer appears now if you head left out of the cafeteria, and you can buy five packs of smokes from him for 100 bucks total.
#7, Dog Smokes - Buy from Gomer for 10 bucks.
#8, Black Death - Buy from Gomer for 15 bucks.
#9, Charlie Chocolate - Buy from Gomer for 20 bucks.
#10, Prate - Buy from Gomer for 25 bucks.
#11, Roulette - Buy from Gomer for 30 bucks.
Now head all the way back to Centurion to beat down Sawtooth for your next pack of smokes.
#19, Starlife - Beat Sawtooth.
Talk to Centurion and he'll tell you to challenge Cusa in Section D for your next fight. But first, stop by Section C to complete quests for The Nurse and Gulag.
#32, Old Timer - Complete The Nurse's mission.
#3, Carl's Blend - Complete Gulag's mission.
When you reach Cusa in Section D, talk to him to get the ball rolling on the fight and then find Twotongue. Speak to Twotongue to finish his mission for another pack of cigarettes.
#55, Lung Busters - Complete Twotongue's mission.
Twotongue will give you another mission to get him a moth, but you want to collect 10 of them for a pack of smokes. There is no context button on the moths, but you have to get close and hammer . Once you have 10, you can trade them and 10 bucks for another pack of cigarettes.
#22, Painful - Give Twotongue 10 moths and 10 bucks.
You'll need 20 more moths later so you might as well collect the rest in the Recreation Area before heading back into Tower 17 to fight Cusa. There should be 18 moths total (10 for the first pack of smokes, 8 toward the next pack.)
#20, Clemens - Beat Cusa.
Talk to Centurion to arrange a fight with Bam, then you'll have to fight Abbot. After killing Abbot, head back to Centurion in Tower 17 and talk to him to get another pack of cigarettes.
#21, Forty Six and Two - Go back and talk to Centurion after beating Abbot.
Now, head into the Recreation Area and all the way through to the end of Section D to end this hub area nightmare. You will still have the Blueskins and Wilkins quests but you cannot complete these without going through the mining area ahead.
CHAPTER 18 - WORK PASS
#34, The Dweller - Once inside the Work Pass area, go into the door directly in front of you and the cigarettes are inside. I think whether or not the alarm goes off (and the mech chases you down) depends on whether or not it sees you entering or leaving the room.
CHAPTER 19 - MINE ENTRANCE
#38, Style - While on the elevator roof, find the ductwork with the ventilation hatch you can use. Exit out into the hallway, kill or avoid the guards, and stick to the right side until you see another vent above you to jump into. When you reach the first junction inside this vent, the cigarette pack is in the section to the left.
CHAPTER 20 - SECURITY CHECKPOINT
#39, Mr. President - When you're in the security checkpoint with two guards and a riot mech while completely unarmed, you want to climb to the upper walkway above where the mech spawns in this room. Once you're up here, turn 90 degrees right of the door used to exit this area and you'll see a tiny ledge along the wall. You want to jump onto this and follow it to the middle of the ledge between the two doors, then jump up to get your cigarettes.
CHAPTER 21 - UPPER MINES
#40, Space Jockey - When you reach the highest point of this area, you'll come across a door that needs a keycode. If you're standing with this door at your back, head through the door on your left and then through onto the balcony on your right. Quickly go out and to the right to collect the cigarettes.
Make sure you kill the blueskin near the keycard door as well.
CHAPTER 22- CARGO TRANSPORT
#42, Addictive Mayor - Immediately after entering this level you'll see a pair of ore elevator shafts and a med station on the wall between them. The cigarettes are along the opposite wall from the elevators.
If you're confused which direction to go like I was, ride the ore elevators down.
CHAPTER 23 - MINING CORE
#4, Swift - You will be in a tiny area with a forklift and a technician milling around it, as well as a riot mech stomping around. If you go past the forklift and jump into the tunnel, head left and a pack of cigarettes is simply lying on the ground here.
There are 8-10 moths around this forklift that you need to trade Twotongue for another pack of smokes. Hang around until you collect a few. They always spawn in pairs, so if you have e.g. 7 there will be another one somewhere.
#41, Jimboro - After following/evading the mech for a while, you'll come to a large cement ramp leading out of this area that the mech will not go onto. Continue past this ramp and head behind the shipping containers on the ground here, and the pack of cigarettes is behind them.
This area where you found pack 41 is one of your last chances to collect moths.
Just after exiting the freight elevator in the next area, this is your last chance to collect 20 moths for Twotongues. Make sure you've got them.
Head up the nearby ladder and two blueskins will attack you. Kill both and one drops the other half of the list of rats you need for a sidemission. Head back through the same keycard door to Cargo Transport that you used earlier (behind the two blueskins that attack you here.)
Ride the ore elevators up and head through the door. Climb the boxes at the far end of the room and follow the ductwork around to a ladder to get a code from Valya. You'll need this a bit later. Go back from whence you came and jump off the ductwork to go through the ventilation shaft at ground level on the other side of the room to go right back to the elevator you entered this area with. Jump down into the elevator this time.
#37, Sicher - Inside the small room with the health upgrade station at the beginning of the mine levels. Climb the large yellow crate at the opposite end of the room and the cigarettes are on top of the small red and white medical supply crate.
#36, Tiny Tim's Blend - When you take the elevator up with the code Jagger gave you, you will end up on the opposite side of the door from the mech you met right at the beginning of the mine levels. There's a grate at the opposite side of the room from the elevator doors that contains this pack of smokes.
#23, Cone Puffs - Give Twotongue 20 moths in Section D.
#24, Five Fingers - Talk to Jamal Udeen in Section B.
#17, Sensei - Give the snitch list to Cuellas in Feed Ward.
CHAPTER 24 - SECURITY RESEARCH 1
This area and the next are not in the original version, which is why most existing guides only list 59 packs of cigarettes and hence will not get you this achievement when followed. The following three packs of cigarettes only appear in the remastered Butcher Bay campaign on the Dark Athena disc.
#60, Flare - Shortly after stealing a riot mech, you'll run into an enemy riot mech. In this room is a foot soldier behind some glass with a big red button and at the other end of the room is a door labeled Briefing Room. Inside are some foot soldiers behind glass and a smaller room beyond named Computer Room, and in there is your pack of smokes.
CHAPTER 25 - SECURITY RESEARCH 2
#61, Diamond - After progressing to Section E13 with your mech, you'll come to a room where you'll have to exit your mech and use an overhead hangrail to open up a door to continue. After doing so, an enemy mech will spawn below in front of the dooor you need to use to exit. Just inside that door on your right is glass you can break with the mech cannons, and inside there is a pack of cigarettes.
#62, Smog Hog - When you get to Section E15, you'll have to kill a couple of heavily armored robots that the voice on the intercom calls Flamingos. Once you've dealt with them and the infantry way up overhead, hop out of your mech and head directly against the wall at the base of the stack of boxes, then climb up onto the red and white one at the left corner. You'll see the cigarettes up ahead along the wall.
#43, Iron Lungs - You'll be directly over the cafeteria area in Feed Ward where you poisoned Bink (he should still be on the floor moaning here, actually) and your options for exit are a hallway and a ventilation shaft. Take the ventilation shaft first. At the other end is a working door with a big red button behind it and a broken door with a pack of cigarettes lying in front of it. You want to go back through the vent and continue through the hallway as opposed to pushing the button here.
CHAPTER 27 - CONTAINER ROUTER
#44, Carl of Alderholm - Beneath the shipping container you begin this level standing on is a ladder that leads underground. When underground, take the first ladder on your right to climb up to the other container track and find the cigarette pack behind you.
#45, 20 Musketeers - Right after the last one, go up the other ladder at the end of the underground area and you'll see yet another ladder along the wall to your left. Go down this ladder and head all the way to the other end of this crawlspace to find some cigarettes. When finished, go back to the ladder and head all the way up it to continue.
#46, Robot Smokes - You will come across a technician with a red key card and then a small yellow room with a med station in it. If we assume the med station is on the north wall, head to the lone medical crate at the southwest corner and hop on top of it, then jump up into another area above that. You should be standing on your pack of smokes.
#48, Maestro - After coming across your first laser tripwires in the game, you'll com to a very tall room with a door marked "Control Station B". To the right of this door is a ladder that you want to climb and the enter the ventilation hatch up here. Continue through the vent and down a ladder to get your smokes, then head back to the tall room from whence you came.
#47, 68th - Shortly after the last pack you will have to go through the large door marked "Control Station B". There are two laser traps in this hallway, and directly above the second one is your pack of smokes.
CHAPTER 29 - ABANDONED EQUIPMENT CENTER
#51, Cloud 17 - Shortly after collecting your shotgun and grenades, you'll be in a large cavern with a narrow cement walkway on the right and a cliff on the left. Climb the cliff and continue this way until you find a ventilation hatch, then climb through to the next room. There's a stack of crates directly in front of you and to the right hiding a pack of cigarettes.
#49, Mummy Lights - After finding the power cell charging station, you must turn on the power at a nearby service panel to open the door. Go through here and climb up on top of this walkway using the medical crate in the middle. Continue to the opposite end of the walkway up here to get your cigarettes.
#50, Count Gunther - Almost immediately after the last pack, you will get to the end of the walkway and go into a small room with a fan above and a door on the left. Go into the door and there is a ton of goodies in here, including a collectible, an experimental assault rifle, grenades, a boatload of ammo, and the power cell you need to complete an objective. Get the power cell last though, since it triggers a bunch of enemies to come kick your ass.
CHAPTER 30 - CENTRAL STORAGE
#52, Enfermo - Right at the beginning of the level you'll have to use a hangrail to continue. Go left from where you found it to get to another platform with a ladder, and take the ladder up to find the cigarettes atop the walkway.
CHAPTER 31 - LOADING DOCKS
#53, Graes - About halfway into the level you'll come to a room with four infantry enemies inside -- two on ground level and two on upper walkways -- and a large forklift in the center. Up on the walkway that the forks on the forklift are pointing at is your pack of smokes.
CHAPTER 36 - FACILITY CONTROL
#54, Gawd - After getting your first gun (from an enemy that you can only kill in a brightly lit area with your fists, joy) you'll go through another orange service tunnel like the one you just came out of. You'll go up some stairs and be in a large hallway. Across the hall from where you entered is a small room that contains your cigarettes.
CHAPTER 38 - TAKEOFF PLATFORM
#2, Candy Candy - When you reach the final room with two infantry and an enemy heavy mech, killing all of these will make your mech stop functioning regardless of damage level. On one side of the room is a higher level you can climb onto, and up here is one of the final two packs of smokes in the shadows.
#14, Space Cowboy - At the opposite end of the room from the last pack, by the red and white medical crate, is your final pack of cigarettes.

Thanks for making this, looking at other guides was a pain. One important note though, you have Gronkos Bar labeled as pack 32 when it's actually pack 31. You do correctly have pack 32 labeled later on in the guide though (Old Timer, complete the Nurse's mission.) Thanks again!

Good catch. I've fixed it so that Gronkos Bar is correctly labeled as #31.
This game has been out a long time. Therefore there are plenty of guides out there. Unfortunately, most of the sites I have found and used over the years are shutting down some pages and, therefore, some splendid resources have become unavailable. In the hope that they some day might work again, I'll leave the links stricken through. These resources include;
You do not need to gather all packs at once. You can go back to checkpoints, redo missions and even switch difficulties. All packs will be saved as soon as they are found and the game had a chance to save the progress which occurs right after you have been presented with the package view.
Note: This version of the game includes a few collectibles that were not present, or could be found in different locations, than in the PC version. If you use those guides, be aware of the issue.

Packs 60 to 62 are outlined here. It is part of the guide!
The link is to the page containing all collectibles. It just happens that they listed the newer content first. On the list to the right you can easily navigate to the Escape From Butcher Bay collectibles. Here is the link to the first area of the old game.

Ahh I see it now - Comment deleted and changing my vote to positive. Just a bit of advice the web page displays differently dependant on the browser/browser settings (I used Android Browser and IE at 1680 x 1050) and hence the links at the side can be obscured or appear off the bottom of the screen meaning that the bounties for Dark Athena appear first and it therefore seems as if the wrong link has been posted.

Thanks, I followed the other solutions links and it wasnt for the xbox360 so missed a few collection pieces, now I get to play back through to finish em up. Will look through these links and hopefully find em

You can replay from individual checkpoints and don't have to replay all of the game. I would suggest you make sure it wasn't a mission reward first. If it wasn't, could you tell me what number you are missing, I might be able to help!
For cigarette packet #12 in the infirmary. Before you leave the cell blocks make sure you speak to Shabby. He is near the exit of the cell block that leads to the infirmary. He will give you the code not the guard in the room like all the other guides on here say.

Because I had to Search google, thought id mention here. Cigarette Pack #30 To get the Vent tool turn right out of the showers then left into a room go into back left corner jump down where the grate is missing behind a small guide rail and the vent tool is on the floor.
There are 62 total cigarette packs. This does not have to be done all in one playthrough. Once you complete the Escape from Butcher's Bay campaign, you can reload any checkpoint and grab any cigarette packs you missed. As soon as you get a cigarette pack, it is permanently saved; you do not have to get it again if you die.
However, the best way to get these is to use a walkthrough that details where each pack is, doing it in conjunction with Bell Boy. This is because you can only get some packs if you do all optional objectives in the game. You have to buy packs from some of the prisoners or do missions for them to get other packs. Reloading to do that is not optimal but it can be done.
