I managed this on the very first level (Steel Mill-Pressed into Action) on Casual.
When you get to the first set of overhead walkways outside you're forced over towards the processing plant with the carts being pulled up and down. One enemy runs across the background here, I got it for shooting him.
I don't think I had a massive multiplier x5 maybe although x10 is quite doable by this point.

Go on insane get 5-10 kills and should unlock soon I recomend the first manor house event and get long range head shots

Venice - level 6. Do the first round killing spree and when you arrive at the beginning of the second round, go left and down. You'll see a bonus enemy running in the tunnel, kill him and bingo!
And if you keep going shooting enemies, you can also achieve: Tabletopper - Get in the top 5,000 on any leaderboard in Tournament.

I got this without even realizing it on the 1st level. Just build up your combo (see "Rack 'Em Up" for tips) and hit one of the bonus enemies or harder enemies.
