This is basically the bad ending that requires you to finish the game without getting the Sanctified Moon Incense
There are two endings, Ending A should be your first run, Ending B should be your second run where you go for the permanent damage achievements as well as just speed running through the game without doing any side missions so you won't get the three items you need, (Moon incense, Blood ruby, Holy water of restoration) to create the Sanctified Moon Incense.
