This is awarded for getting the 'good' ending for the Psychiatrist. 'The Psychiatrist' isn't a game of it's own, but rather a result based on your decisions in the four mini-games.
For any of the psychiatrist endings, you must first finish each of the four games (Watchful Gaze, Lord of the Road, Red Button, and Nocturne). When you complete each game, you will have a checkmark next to that game (or session). If the checkmark is green, it indicates you got a good ending. If the checkmark is red, it indicates a bad ending.
For this achievement, you must simply beat all 4 games (sessions) with more green checks than red ones.
Refer to the specific ending achievements for each mini-game to figure out how to get the results you want.
