How to unlock the Six Minutes of Night achievement in The Dark Pictures Anthology: House of Ashes - Definitive Guide

Missable achievement - you have to fulfill several conditions
(If you do not want any spoilers stop reading here!)
I will describe my way to do this achievement
(You have to remember that all characters must die during the last fight in a small house on the surface, in "Little Hope" and "Man of Medan" they could die anywhere but in "Hause of Ashes" the need to die during this "Last man standing").
1. All characters must survive and reach the chamber with strange antique musical instrument in the middle! (I described many possible deaths of characters in guide for "Golden Path" achievement so check it out)
2. Rachel can die here, after she helped Salim with instrument, its how you can save her -
I - When she will want to detonate herself with knife and grenade do not press anything!! Time will pass and she will give up with this idea.
II - While Eric and Nick are holding here you have to choose "Do not kill me" and "Its me Jason" when Jason is aiming in Rachel's direction.
III - This will eventually end up with a fight with Jason (You have to press
a few times in order to disarm him). After you do that you have the last choice - you have to agree with cocoon idea.
IV - They will move you together and Eirc eventually will take out his UV wand and will force this parasite / alien out of Rachel's body.
3. Nick is next, Nick will go plant the explosives, he will have two choices "Advance" and "Retreat" you need to choose "Retreat" if you do not it will end up in kindda dangerous situation and there is a high risk of losing your character.
4. Jason needs to recuse Samil and reach the elevator with rest of teammates
5. When they start climbing the rope, Eric will see Infected-Clarice, she will attack them you must complete QTE (QTE is a minigame which requiares to press
, 
, or 
and 
or 
). If you mess it up you will lose Eric or Rachel/Nick.
6. All characters will make their way, finally they will reach the small house I decided to mess up all QTEs, just do not press anything while fight and everyone gonna die

Just a note on this. When I reached the finale and started failing the QTEs before the ancient character storms the house, the game would inevitably crash every time before the scene finished (usually during one of the character deaths). If you run into this bug, there is a simple solution...
Kill the ancient character using Eric and Rachel, and pass all QTEs until each of the five characters stand back to back, lighting flares for the final standoff. At this point, FAIL all the QTEs to see them die in a number of brutal ways. Once they are all dead, the vampires head back into the depths and the eclipse finishes. When the helicopter turns up, the achievement should pop.
Good luck!

Rachel died from Clarice when she attacked. I killed everyone else off in the end house and got the achievement.

Following this video guide I put together will show you how to keep everyone alive throughout the game and once you reach the finale all you need to do is miss all of the quick time events to get everyone killed to get this achievement, simply follow my choices.

Can anyone confirm if Balathu must also die in the finale for this? (The achievement description implies that he does).
Experienced a weird glitch, where if I got to the finale with everyone and just pressed nothing, the game would freeze and kick me back to the menu.
The only way it would let me play through and unlock was if I failed the first QTE (Nick gets dragged through the roof) and the passed the QTEs for Rachel and Eric (They kill Balathu) and then failing all the rest (Vampires kill everyone).

When the trophy says "everyone died in the finale," they really mean it. In order for this trophy to unlock, you'll need to get all five main characters to the eclipse battle; having fewer than all five characters reach the finale will void the trophy. From there, you can safely fail each of the QTEs, resulting in a gruesome death for each of them. With everyone dead, the trophy will unlock shortly after.
