the easiest way to get this is to keep everyone alive then get to the boat at the end then fail all the QTEs then you can reload and get the achievement for saving everyone
Deaths in the story:
First death can be Erin. While exploring the halls with directional microphone, you'll eventually reach the dark room. When Erin is in the dark room and Du'met appears, attack him twice (QTE). Doing so will cause Du'met to end Erin in the room.

As of 12/13/2022, version 1.03, when you select the “Attack” option on the boat, the game freezes, well, at least for me. Reinstalling the game or rebooting the console does not help. To get a bad ending after a good trophy, you need to load the “Lake” chapter, put down the gamepad and go drink tea and cookies for the entire chapter. If decision timers and QTEs were disabled in additional settings, you need to enable them.
