Once you have the breathing potion, use it on yourself, and go to the lake, then to "Lake Bottom" (just below the dock).
Once there, "use" the sword to get it in your inventory. Then, go back the use village and down the well. "Use" the sword on the door here to open it. The achivement will unlock once you enter the "Dark Passage".
