At some point in the story (In the brothel) you are forced to choose between 1 of 2 people on a TV screen via Lt or Rt.
Simply do not press either and after a few seconds it chooses for you.
And Achievement unlocked

You'll come to a point, in the game, where you have to choose between two men to be executed. As long as you don't press any of the buttons for a few moments, the game automatically continues on and the secret achievement "Decisions, Decisions" should unlock right then and there. No problem! Have fun and good luck!
