Story related - you’ll fight a salamander at the end of the first yellow story mission of chapter three “escort the priests”. Right before depleting it’s first health bar, try moving away a bit as it will burn all nearby characters when the first bar depletes.

This was about nine hours into the game for me, doing all side missions and cutscenes. Maybe seven if you’re rushing through.
