After loading into a prison (Center perks 2.0 is probably the easiest) take off your bed sheet by holding 
next to the bed then go into another inmates cell and hold again on their bedsheets. After you get both bed sheet go into your inventory using 
and scroll over once using RB. Under the 30+ intelligence category you should see a sheet rope, craft it with your bed sheets and the achievement is yours.

A big part of this game is crafting useful items from basic items you will encounter around the prison. You have a crafting menu which shows you the majority of recipes for crafting an item. Be aware that you need to have a certain level of intellect to craft some of the more complex items. Your intellect can be increased by reading books from any bookcase in any prison.
