For this achievement you need to complete all 3 anima events. The first one being south of the pit stop in a house in chapter 3 . The 2nd one is in southern business district in jukes diner in chapter 7. Interact with the dead man in the chair to start this one. The final one is chapter 11 in the marrow restricted labs. For the final one you need to open the shutter in the morgue area. From here go down the hallway and enter the door on the right. Pick up the document on the desk. After completion of the third event you'll get the revolver and the achievement will pop.

To get this trophy, you must survive at three different encounters with a mysterious entity that will chase you.
The first time you will meet her is during chapter 3, in the residential area, inside "336 Cedar Av." house. By taking the "Woman's Journal", you will be teleported in a new scenario. The entity will hunt you and you have to find a Key Card to exit. The key card is in the last room, inside a locker. Use the key to open the door on the first room. The second event is in the Business District, inside the Juke Diner kitchen in Chapter 7. By watching the Residual Memory "After Life" you will find yourself inside a room with a mirror and 4 doors. Looking in the mirror, you will see one of the door with light. Go inside that door and you will go on. Do this on another room and you will be out. You will find a slide once you're free.
