During Chapter 3 you are required to clear out the enemies and get a gate open for the Doctor. Once you have cleared out the enemies you need a chainsaw to get the gate open and you have to get this off the Sadist who is trapped inside the barn. If you look around enough you will find explosive bolts and a crossbow which will help you kill him.
To get him to appear you need to walk up to the area he is trapped in and he'll break free and begin chasing you. So equip the crossbow and shoot him straight away with an explosive bolt and then make your way out of the barn and to the left and go left of the gate into the door, there will be a trap on the floor and as he's following you, run up the stairs and trigger the trap and this will kill him netting you the achievement.

In chapter 3 you have to clear through the small town outpost and defeat the sadist to acquire his chainsaw to cut through the chain on the gate locking you in. Once you clear the whole area of enemies keeping in mind to stay out of the ground floor of barn which triggers the sadist to break free, you should have acquired at least a couple explosive bolts and grenades. Go up the stairs on the exterior of barn to 2nd floor using your crossbow with explosive bolts set one bolt in front of the doors the sadist breaks through and another bolt at bottom of ladder that leads to 2nd floor. After explosive bolts are set in position on the 2nd floor goto railing over sadist now chuck all your grenades below damaging him and triggering his escape now sit back and watch him set off your explosive bolts he will be dead in 13 seconds on survival mode by the time he reaches the ladder. Afterwards claim your chainsaw and chest of ammo from where he was chained. Hope my strategy for the sadist helps.

If anyone is really struggling to kill him, I did this tactic on Survival difficulty. I would not recommend it as your first attempt, this is a last resort if you are very low on ammunition or really having trouble with him.
Okay, so I used the shotgun to shoot down the wooden door in front of him, then while he was reaching to grab his chainsaw which takes a good few seconds, I swapped to my grenades using the quick access feature.
I threw three grenades at him and he was only just about to chase me by that time so aim and throw them as fast as possible. Then I swapped to the Agony Crossbow and used an explosive bolt, firing it onto his body. Make sure he is not too close to you because it will blow you up too.
Once that exploded he went down. Don't forget to go inside where he was chained to open up the chest that was behind him.
This method is wasting all of your grenades so only really try this if you're stuck. (I had hardly any ammo and very low health.)
